This is a Moisture and Microbial Remediation Guidelines document prepared by EMS for Halifax Health's Child and Adolescent Behavioral Services facility located at 841 Jimmy Ann Drive, Daytona Beach, Florida. The document, dated December 5, 2025, provides comprehensive specifications for the remediation of the 200 Wing following moisture intrusion and microbial contamination identified during an assessment conducted on November 3, 2025. The project requires a remediation contractor to perform containment installation, drywall and insulation removal along flood cut lines, structural component evaluation and repair, HVAC system cleaning and sanitization, soft goods and hard goods remediation, demolition and disposal of impacted materials, and final surface cleaning and sanitization. The remediation must comply with IICRC S500, OSHA, NIOSH, EPA, and applicable local and state regulations. All work shall utilize specified Bioesque products for disinfection and encapsulation, including Bioesque Botanical Disinfectant, Bioesque Mold Resistant Coating, and Bioesque Protect 90. The project includes secondary containment isolation for eight patient rooms and one therapy room within the primary 200 Wing containment, with negative pressure maintained at minimum 0.02 inches of water column until post-remediation air verification sampling confirms acceptable conditions.

The remediation contractor is solely responsible for worker protection, facility staff safety, project scheduling, work area security, waste material management, and containment integrity throughout the project duration. The contractor must maintain HEPA-filtered negative air scrubbers with documented operational verification a minimum of three times per eight-hour shift, change HEPA filters daily in non-demolition areas and twice daily during dust-generating activities, and perform a 24-hour air recirculation scrub cycle prior to final post-remediation verification sampling. All impacted materials, including drywall, insulation, ceiling tiles, and cleaning wipes, must be bagged in 4-6 mil polyethylene and disposed of appropriately. Final clearance requires spore trap and/or swab sampling analyzed by an NVLAP, AIHA, or A2LA accredited microbiology laboratory, with results demonstrating acceptable air quality before the contained spaces may be released for occupancy. The document includes an attached Impact Map dated November 3, 2025, identifying specific areas of elevated moisture, impacted materials, and the recommended containment location for the 200 Wing.

25217.00 – Halifax Health 200 Wing Remediation Guidance Document Halifax Health Child and Adolescent BHU – Daytona Beach, FL December 5, 2025

These remediation guidelines are based specifically upon the onsite visit and photo documentation collected by EMS on November 3, 2025, and are not intended to take into account the changing conditions within the subject property after this date or conditions in other potentially impacted areas of the facility.

EMS has no firsthand knowledge as to the extent or completeness of any prior building repairs, moisture intrusion, remediation, and/or clean-up efforts that may have taken place at the property or within these target spaces prior to this date. These guidelines are also not intended to address asbestos, lead, or other environmental, health, or safety hazards.

Prior to the implementation of these guidelines, sources of moisture intrusion and/or microbial sources are to be addressed and corrected to prevent further impacts, building material degradation, and microbial amplification. Corrections and repairs are to be made by the appropriate Florida-licensed trade professional(s).

These specifications apply to all individually phased containments. Project phasing for the execution and completion of this project should be defined and agreed upon by the facility, the remediation company, and the consultant. All containment and critical barrier installations should be reviewed by the consultant prior to the commencement of remediation work.

Containment is understood to be the areas where remediation work will be performed, and air clearance activities will take place. The containment is to remain in place and under negative pressure until acceptable post-remediation air verification sampling results have been achieved.

The contained spaces are to remain vacated by the general public, employees, patients, and any personnel who are non-essential to the remediation activities until all remediation activities have been completed and verified through final air verification sampling.

EMS recommends the following products be used, as applicable, for the execution of this remediation guidance:

Bioesque Botanical Disinfectant - to clean and disinfect all surfaces. No rinse is required on food-contact surfaces. It kills mold, viruses, and bacteria (including tuberculosis). EPA-approved to be used with ULV foggers and electrostatic sprayers. EPA-approved to be used in HVAC systems, including ductwork.

Bioesque Mold Resistant Coating - to encapsulate. This product has four EPA-approved ingredients added to help prevent future mold growth and is Lubrizol certified.

Bioesque Protect 90 - to fog. It is safe to use on hard and soft surfaces to protect from mold, fungus, algae, bacteria, and odor-causing bacteria growth. EPA-approved to be used with ULV foggers and electrostatic sprayers as a final step to remediation before clearance testing. EPA-approved to be used in HVAC systems.

The supply of PPE equipment and associated training of employees in the use, maintenance and limitations of the equipment is the sole responsibility of the remediation contractor.

If any requirements, notifications, and/or regulations overlap among or between the above references, the most stringent provisions shall apply.

Containment(s) and Critical Barriers

Based upon the data collected during the on-site assessment, it is recommended that the entirety of the 200 wing be placed under a negative pressure containment. Additionally, it is recommended that each patient room and the therapy room be placed under secondary containments that isolate these spaces from the rest of the wing.

Containments are to remain under negative pressure throughout the duration of the project until post-remediation verification testing confirms that the spaces are safe, containments can be removed, and the spaces can be released for continued occupancy.

Containment

• Containment walls are to be constructed of fire-retardant 6 mil. polyethylene sheeting or edge guard hard panels.

• Using fire-retardant 6 mil. polyethylene sheeting, erect a floor-to-ceiling containment anteroom.

Due to the special security considerations specific to this facility, the anteroom may be located immediately inside the double door entry to the wing from the main corridor.

• A zip door is to be installed at the entry to the wing from the anteroom. Additionally, a zip door is to be installed at the entry to each patient room and therapy room.

• All containment and anteroom doors and barriers, as applicable, are to be sealed tightly at the ceiling and floor.

The RC shall use fire-retardant 6 mil. polyethylene sheeting to install critical barriers to isolate the entirety of each target space from possible contamination sources. These include:

• Adjacent spaces

• HVAC supplies/returns

• Pass-through wall vents

• Outlets and switches

• Breaches in the exterior wall envelope

• Open drains

• Above-ceiling spaces

• Exterior windows

Zip doors, edge guard panels, containment barriers, and critical barriers should be inspected regularly to ensure integrity is maintained.

Foot Traffic

The RC shall restrict traffic entering and exiting the contained area to only the necessary remediation personnel during all remediation activities and will ensure that all travel paths for materials and waste debris remain clear for the duration of the project.

The RC shall utilize sticky walk-off mats inside the entry to the containment from the anteroom to control footwear dust and dirt tracking. Walk-off mats are to be inspected regularly for dirt loading and changed as they become laden with dirt. Mat edges are to be secured to the floor to prevent a tripping hazard.

• New shoe coverings are to be donned upon each entry into the main containment and are to be removed and discarded upon each exit.

Negative Pressure Specifications

HEPA-filtered negative air/scrubber machines equipped with GFCI overcurrent protection shall be used to provide negative pressure to the contained spaces. The effluent air shall be exhausted to the exterior of the building.

• A minimum of -.02 inches of water column pressure differential relative to outside pressure is to be maintained within the negative pressure containment(s).

• A differential pressure recorder with data-logging capability is recommended. If unavailable, a number of negative air machines are to be used that ensure a minimum of 6 air exchanges per hour by calculation. Visual verification that all scrubbers are operational is to be documented a minimum of three (3) times per eight (8)-hour shift and spaced at least two (2) hours apart.

• During demolition or dust-generating activities inside containment(s), HEPA filters and pre-filters should be changed a minimum of twice daily.

• In areas where non-demolition or dust-generating activities are taking place, HEPA filters and pre-filters are to be changed daily.

• Negative air/scrubber machines are to be cleaned by vacuuming and antimicrobial wet wiping before new filter installation.

HVAC System

The HVAC system servicing the unit is to undergo cleaning and sanitization. This should include spiral fans, drip pans, interior air handling units, coils, gasketing, and supply/return grills. Additionally, any interior-lined ducts or duct board is to be encapsulated. Once complete, the system(s) should be isolated from areas undergoing remediation until remediation has been completed.

This work shall be completed as per the National Air Duct Cleaners Association (NADCA) cleaning standard ACR 2013 - Assessment, Cleaning, and Restoration 2013.

Halifax Health Child and Adolescent BHU – Daytona Beach, FL
