About this file

This is a Non-Conflict of Interest affidavit form for offerors submitting proposals to the Colorado Restorative Justice Program contract opportunity. The document requires the offeror or authorized representative to certify under oath that they have no conflicts of interest that would prevent them from performing the requested services. The form establishes the legal framework for ensuring competitive integrity and compliance with state procurement regulations by documenting the offeror's attestation regarding potential conflicts with the contracting entity, other bidders, or project stakeholders.

The affidavit must be completed and signed by an authorized representative of the offeror, with the signature notarized as required by Colorado law. This certification document serves as a mandatory submission component of the proposal package and confirms the offeror's understanding of conflict of interest prohibitions and their commitment to ethical business practices throughout the contracting process. Failure to provide this completed form or misrepresentation of conflicts may result in proposal rejection or disqualification from award consideration.
