About this file

This is a Non-Conflict of Interest attestation form required by the Colorado Department of Corrections for bidders participating in the Boiler Chemicals & Treatment Services procurement opportunity in Colorado. The form requires bidders to certify compliance with Colorado Revised Statutes sections 18-8-302, 18-8-306, 18-8-301, and 18-8-308, as well as the State of Colorado State Controller Policy on Procurement Conflicts of Interest. Bidders must confirm they have no actual, potential, perceived, or apparent conflicts of interest related to the procurement and acknowledge that the CDOC Procurement Official has authority to remove them from the process at any time.

The form establishes that bidders understand they may have access to State information and records during the procurement process, and that such information is custodial under the CDOC Procurement Official and cannot be distributed, sold, shared with third parties, or used except as expressly authorized. Bidders must commit to immediately notifying the Procurement Agent if any situation arises that could alter their representations or create the appearance of conflict or impropriety. The form requires signature and authorization by an authorized representative of the bidding entity and must be returned with the bid submission. This attestation applies to both private employer employees and independent contractors under contract with the State of Colorado.
