About this file

This is a Cost Proposal Form for the State of Utah Department of Commerce's Web-based Biological Specimen Monitoring Program (JK26-40), which seeks to establish a single contract award for an electronic health compliance monitoring, tracking, and toxicology testing platform serving probationers and participants. The procurement is anticipated to result in a 5-year contract term with pricing guaranteed for the initial year. The RFP requires vendors to submit detailed cost proposals for specimen collection, laboratory analysis, and related administrative services as outlined in the Scope of Work (Attachment C, Appendix A).

The Cost Proposal Form requires offerors to complete all yellow-highlighted fields specifying fees for: vendor administrative costs per specimen; routine collection site fees per specimen; remote collection options including collection kit costs, return laboratory shipping, and collection/observation fees; and routine laboratory fees per specimen for three distinct toxicology panels—UPHP (D-76404) for Medpro A, ETG, and Fentanyl screening with GCMS confirmation; Naistat 12 Drug Panel with toxicology screening and confirmation; and PEthStat with toxicology screening and confirmation. Vendors may submit additional service fees outside the evaluation formula, which will not be scored for cost points but may be incorporated into executed contracts. The total calculated fee from the standardized format will be used for cost evaluation purposes according to the formula specified in the RFP.
