About this file
This is a Scope of Work document for the Utah Department of Health and Human Services (DHHS) Blood Spot Newborn Screening program, establishing requirements for a capillary zone electrophoresis system to screen for hemoglobinopathies in dried blood spot samples. The contractor must ship two CAPILLARYS 3 DBS instruments with associated UPS systems and processing computers, install and configure the equipment, perform instrument validation for each platform, and provide comprehensive training to up to two DHHS newborn screening laboratory staff members. The contractor is required to maintain a forty-eight-hour emergency repair response time for Monday through Friday service requests and deliver two preventative maintenance visits per calendar year for each instrument. The contract enables DHHS to identify normal hemoglobins (Hb A, F) and abnormal hemoglobins (Hb S, C, E, D, Bart's) in newborns, supporting early preventive measures or treatment to improve quality of life and reduce infant mortality for all individuals and communities throughout Utah.
The contractor must ensure timely completion of service requests and shipment of all necessary reagents for the contract duration and provide subject matter expertise as requested by DHHS staff. Pricing requirements include submission of a price quote for the entire contract period with unit pricing and an estimated annual reagent commitment. The contractor must submit invoices for shipped goods on the first business day following shipment and must include standard invoice documentation such as contractor name, payment address, phone number, contract/PO number, invoice number and date, and service details. All invoices for services performed on or before June 30th of a given fiscal year must be submitted no later than July 14th of the following fiscal year, aligned with Utah's fiscal year running from July 1st through June 30th. The contract is structured as a five-year agreement for rental and reagents for the Capillary 3 DBS system, with the state indicating intent to award without engaging in standard procurement based on single-source justification.

The purpose of this contract is to enable DHHS Blood Spot newborn screening (“NBS”) staff to screen for life-threatening Hemoglobinopathies using dried Blood Spot (“DBS”) samples utilizing capillary zone electrophoresis for the identification of normal (Hb A, F) and abnormal (Hb S, C, E, D, Bart's) hemoglobins.
Article 2
DEFINITIONS
2.1 Definitions. In this contract, the following definitions apply:
“Blood Spot” means a clinical specimen submitted on the paper of the NBS form using the heel stick method.
“Capillary Zone Electrophoresis” means an analytical technique that separates charged molecules inside a narrow tube using an electric field and a buffer solution and sorts of particles into distinct zones based on their electrical charge and size
“Hemoglobinopathies” means any conditions caused by changes to hemoglobin that affect the shape or amount of red blood cells in the body in an unhealthy way.
Article 3
SERVICE REQUIREMENTS
3.1 Services. The Contractor shall:
(1) ship two CAPILLARYS 3 DBS instruments, two UPS systems and two processing instrument computers;

Hemoglobinopathies using DBS samples and utilizing capillary zone electrophoresis for the identification of normal (Hb A, F) and abnormal (Hb S, C, E, D, Bart's) hemoglobins.
Identifying newborns at risk for Hemoglobinopathies is essential for early preventive measure or treatment which can improve quality of life and may reduce child mortality of infant under the age of one year old. Because NBS for Hemoglobinopathies is universal, this service will support and serve all individuals and communities throughout Utah.
