About this file
This is a Sexual Harassment Training Policy Statement certification exhibit required for bid submission to Erie County Medical Center Corporation (ECMCC) in New York. Bidders responding to ECMCC's request for proposals to supply CPAP and BiPAP devices, along with accessories and compatible stands, must certify under penalty of perjury that their organization has implemented a written sexual harassment prevention policy meeting the requirements of New York Labor Law Section 201-g and provides annual sexual harassment prevention training to all employees. The certification must be signed by an authorized representative and affirmed on the specified date.
The document emphasizes the legal consequences of false certification, noting that knowingly making false statements in written instruments constitutes a Class A misdemeanor under New York Penal Law Section 210.45. All bidders are required to complete and submit this exhibit as part of their proposal package to demonstrate compliance with New York State sexual harassment prevention requirements prior to contract award.
