About this file

This document is a "Disclosure of Investment Activities in Iran" form issued by the New Jersey Division of Purchase and Property, pertaining to Public Law 2012, c. 25. The form requires bidders submitting proposals or entering into contracts to certify whether they or their parent companies, subsidiaries, or affiliates have investment activities in Iran. Bidders must check one of two boxes: either certifying no involvement with Iranian investments or acknowledging such activities and providing detailed information.

The document outlines strict compliance requirements, with potential consequences for false statements or incomplete disclosure. These may include rendering the proposal non-responsive, assessment of penalties, fines, or sanctions, and potential referral to the State Attorney General. The certification requires the bidder to attest under penalty of perjury, acknowledge a continuing obligation to disclose changes, and understand that making false statements could result in criminal prosecution and contract voiding. The form must be signed and dated, with the signatory affirming their authorization to make the certification on behalf of the bidding entity.
