About this file
This is a Scope of Work document prepared by R & R Environmental, Inc. for the Utah Department of Government Operations, Division of Facilities, Construction and Management (DFCM), detailing pre-demolition asbestos and hazardous materials abatement at Southern Utah University's Ponderosa Terrace Apartments located at 200 South Dewey Avenue, Cedar City, Utah. The project involves removal of multiple asbestos-containing materials (ACM) across nine residential units, including black sink undercoating (18 units at 2% chrysotile asbestos), textured ceiling materials (approximately 7,134 square feet containing up to 3% chrysotile asbestos), joint compound and wall systems (38,106 square feet containing up to 2.25% chrysotile asbestos), pipe thread sealants (approximately 1,100 square feet), one boiler assumed to contain asbestos, fire-rated doors and frames (11 units), and roofing materials and sealants (approximately 4,500 square feet). The project is divided into two bid options: Option 1 addresses items 1-8 (specific ACM removal); Option 2 addresses items 9-13 (boiler removal, textured ceiling material removal, fire-rated doors/frames removal, co-mingled waste demolition, and universal waste removal including PCB ballasts, fluorescent tubes, fire extinguishers, chemicals, microwaves, stoves, and mercury thermometers). Anticipated start and substantial completion dates are listed as TBD. The document was prepared July 17, 2026, with Mrs. Aubrey Stapel identified as the Hazmat Program Manager.
The Scope of Work establishes detailed requirements for contractor qualifications, safety protocols, and operational procedures. All contractors must possess current Utah Division of Air Quality (DAQ) certifications for workers and company-level certification, and must comply with applicable regulations including Utah Asbestos Rule (Rule R-307.801), EPA NESHAP (40 CFR Part 61, Subpart M), OSHA Asbestos Standard (29 CFR 1910.1001), and OSHA Asbestos Construction Standard (29 CFR 1926.1101). Key requirements include attendance at a pre-start meeting with the project supervisor, daily personnel air sampling for friable removal activities with next-day reporting, submission of abatement project design from an AHERA-accredited Utah Certified Project Designer at least three working days before start, hiring a Utah-licensed electrical contractor, provision of weight and measure certifications for all ACM waste with manifest documentation prior to invoice submission, use of fire-retardant poly sheeting with clear plexiglass viewing windows, continuous cleanup of removed ACM with no unattended materials, on-site chemical toilets for personnel, and use of leak-tight containers in poly-lined, enclosed, locked vehicles for waste transportation. The document includes a comprehensive Unit Price Sheet with 81 line items for unforeseen ACM materials, and Appendix B contains detailed asbestos survey and assessment results from laboratory analysis of 67 samples confirming the presence of chrysotile asbestos in identified materials and establishing their friable or non-friable conditions. The Stage II Invitation to Bid indicates a construction cost estimate of $350,000.00 and specifies that only contractors prequalified under Stage I and appearing on the DFCM Prequalified Contractors list as of the solicitation date are eligible to bid.

Notes:
1- This material appears to exist underground.
2- The joint compound contains 2.25% Chrysotile asbestos (by point count) and is considered by the OSHA as an ACM. The Wall System material as defined by the EPA contains Chrysotile Asbestos in the joint compound with an overall asbestos content of less than 1% (by point count). According to the Federal Register page 542, Wednesday, January 5, 1994, 40 CFR Part 61, "Asbestos NESHAP Clarification Regarding Analysis of Multi-Layered Systems", when joint compound is applied to wallboard, it becomes an integral part of it, forming a wall system. Composite analysis of the wall system is to be conducted. In this case, the wall system contains less than 1% asbestos and is therefore not subject to the NESHAP. The joint compound however is regulated by the OSHA. Removal of this material is a Class II Operation under OSHA, which requires 40-hour trained supervisors, trained workers and specific work practices (wet methods, prompt clean-up of debris, HEPA vacuums, etc.)

The asbestos classification system outlined in the revision and included in this section is dynamic in nature. Asbestos materials classified as “NON-REGULATED” at the time of the survey may become “REGULATED” due to ongoing or planned maintenance, renovation, or demolition actions, which can transform a material containing greater than 1% asbestos from a “non-friable” and NON-REGULATED to a friable and REGULATED condition.
Classification of ACM in this section and in the executive summary of this report is, therefore, based on the observations of the surveyor at the time of the survey and may or may not be appropriate at later dates.
4. Maintenance, renovation, demolition, weathering, normal wear, water, or other damage can alter the “NON-REGULATED” status of materials, and necessitate precautions required for handling them as “REGULATED” asbestos-materials.

3.0 SAMPLING METHODS
Survey procedures were based on those outlined in Title 40 Code of Federal Regulation (CFR) Part 763, USEPA Asbestos Hazard Emergency Response Act, and the Asbestos Model Accreditation Plan (effective April 4, 1994). The procedures included visual observation, physical inspection, bulk sample collection, and condition assessment of suspect ACM. The number of samples collected was based on the amount and accessibility of each homogeneous material, with consideration given for the type, age, and condition.
Sixty-seven (67) samples were collected from the suspect materials to assess the presence of asbestos (or lack thereof) in building materials/systems. Each sample collected was placed into a sealable plastic bag or tub, marked with a unique sample identification code, and packaged for subsequent shipment to the laboratory for analysis. Individual sample identification consisted of a numeric code denoting the sampling media/location.
After packaging and labeling, samples were submitted under proper chain-of-custody to Reservoirs Environmental, Inc., Denver, Colorado. The National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code 101896 accredits Reservoirs for asbestos analysis.
Samples were analyzed via Polarized Light Microscopy (PLM) utilizing EPA Method 600/M4- 82-020. Please note that USEPA and OSHA regulations define an ACM as a material containing more than one (1) percent asbestos, by volume, as determined by PLM analysis. In the case of multi-layer systems (e.g., floor tile/mastic layers), the laboratory analyzed and reported each layer separately.
4.0 SURVEY RESULTS
Sixty-seven (67) representative samples collected were submitted for analytical determination.
A summary of analytical results for samples collected and analyzed for this survey are in Appendix B (Laboratory Results) and are discussed below by building material/system type.
Photographs of the buildings where samples were taken are included as Appendix C. Floor Plans of the building where samples were taken are included in Appendix D

5.0 CONCLUSIONS
Asbestos-containing materials were identified in the building. See Executive Summary for details.
The identified materials must be removed prior to disturbance by a licensed abatement contractor.
6.0 LIMITATIONS AND EXCLUSION OF WARRANTY
This asbestos survey and assessment was performed using procedures and a level of diligence typically exercised by professional consultants performing similar services. However, asbestos-containing material (ACM) can be present in a structure, but not identified using ordinary investigative procedures.
No asbestos survey can completely, eliminate uncertainty regarding the presence of ACM.
R&R Environmental, Inc.’s level of diligence and investigative procedures are intended to reduce, but not eliminate, potential uncertainty regarding the presence of ACM. The procedures used for this survey attempt to establish a balance between the competing goals of limiting investigative costs, time, and building damage, and reducing the uncertainty about unknown conditions. Therefore, the determinations in this report should not be construed as a guarantee that all ACM present in the subject property has been included in this report.
This report presents R&R Environmental, Inc.’s professional determinations, which are dependent upon information obtained during performance of consulting services. R&R Environmental, Inc. assumes no responsibility for omissions or errors resulting from inaccurate information provided by sources outside of R&R Environmental, Inc.
No warranty or guarantee, expressed or implied, is made regarding the findings, conclusions, or recommendations contained in this report. The limitations presented above supersede the requirements or provisions of all other contracts or scopes of work, implied, or otherwise, except stated or acknowledged herein.
