About this file
This document is an Offeror Information and Certification Sheet for a state procurement opportunity in Oregon for a CBO Collective Lead Organization for Child, Youth, and Family Supports. The procurement is seeking an organization to serve as the lead for a collective of community-based organizations providing child, youth, and family support services. The document outlines the certifications and information required from offerors, including compliance with state non-discrimination laws, tax laws, conflict of interest policies, and other state requirements. Key dates, terms, and conditions are specified, including that offers must remain firm for 180 days and that the selected contractor must be able to perform the full scope of work.
The document also highlights specific state laws and regulations the selected contractor must comply with, such as requirements related to broadband internet access service and the Oregon False Claims Act. Offerors must certify they are not on federal exclusion lists and that there are no actual or potential conflicts of interest. The document serves as an attachment to the overall procurement solicitation, providing important terms, conditions, and certifications for prospective offerors to review and agree to as part of their response.
