1. Purpose of Procedure

The purpose of this document is to provide current and pertinent information regarding the steam and condensate systems at Captain James A. Lovell Federal Health Care Center and should be updated accordingly. This procedure provides a step-by-step approach for safely isolating and re-energizing pressurized steam and condensate systems, using a double block and bleed valve configuration, with additional warm-up valves for controlled re-pressurization. The goal is to ensure safe isolation, minimize risk, and maintain system integrity while performing necessary maintenance tasks.

The contractor is responsible for updating general and building-specific procedures and risks whenever they change. For instance, after the installation of new double block and bleed valves, the contractor should include photos and as-built drawings to create or update the building-specific operations and maintenance manuals. After construction, the task of updating and maintaining this document will fall to the department and individual that operates and maintains the system.

2. Safety Precautions

Before performing any isolation or re-energization activities, ensure the following safety items have been reviewed and implemented by all participants:

• Personnel Training: Ensure all personnel involved are trained and qualified in steam systems and valve operations.

• Activity Hazard Analysis (AHA): Implement AHA procedures to prevent incidents.

1. Refer to attachment 3

2. U.S. Army Corps of Engineers Headquarters > Missions > Safety and Occupational Health > Activity

Hazard Analysis

3. Safety Management - Hazard Identification and Assessment | OSHA.gov | Occupational Safety and

Health Administration

• Lockout/Tagout (LOTO): Implement LOTO procedures to prevent accidental re-energization.

1. Refer to attachment 4

2. Control of Hazardous Energy (Lockout/Tagout) - Overview | OSHA.gov | Occupational Safety and

Health Administration

3. 1910.147 - The control of hazardous energy (lockout/tagout). | Occupational Safety and Health

Administration

• Personal Protective Equipment (PPE): Personnel must wear appropriate personal protective equipment, including gloves, goggles, hearing protection, flame-resistant clothing, and a hard hat where required.

• Ventilation: Ensure proper ventilation, including rate of speed, when working with steam systems to avoid exposure to hazardous gases or steam burns.

• Warning Signs and Barricades: Post clear warning signs around the work area, and ensure access is restricted to authorized personnel only.

Safety Risks:

o High Pressure

▪ Steam lines often operate at very high pressures, which increases the risk of equipment failure. If the pressure exceeds the design limits, the steam line or components can burst, causing serious injuries or fatalities.

o Pressure Fluctuations

O&M Procedure VA Project # 556-24-108

O&M Procedure April 2025 4

▪ While condensate lines usually operate at lower pressures, fluctuations in pressure can still occur, especially if there is a malfunction in the condensate pump or steam trap system. Pressure spikes can damage the condensate line or create safety hazards.

o High Temperature

▪ Steam in the lines is often extremely hot (typically ranging from 150°C to 600°C or higher). Direct exposure to the steam or the surfaces of the pipes can cause severe burns or scalds, and if there is a leak, the surrounding environment can become dangerously hot.

▪ Although condensate is typically at lower temperatures than steam, it can still be hot (ranging from 120°C to 200°C in some cases). Direct contact with hot condensate, especially during maintenance or repair activities, can cause severe burns or scalding injuries.

o Leaks and Spills

▪ Leaks can occur due to wear and tear, corrosion, or mechanical damage. A steam leak can cause a sudden increase in pressure and temperature in a confined space, which could lead to burns, scalds, or even explosions.

▪ Condensate lines can develop leaks due to corrosion, physical damage, or improper installation.

A leak can lead to the release of hot condensate, which may cause burns or damage to surrounding equipment. Leaks can also lead to water damage in areas that are not designed to handle moisture.

o Steam Explosions

▪ In some cases, a catastrophic failure of the steam line (e.g., due to an equipment malfunction or rupture) can lead to a steam explosion, which may cause significant property damage, injuries, or fatalities.

o Condensate Contamination

▪ Steam systems typically generate condensate (the water formed when steam cools down). If the condensate becomes contaminated with chemicals or rust, it can pose a health hazard if it leaks or comes into contact with workers.

▪ Condensate lines may carry contaminated water, especially if there are issues with the steam system, such as leaks of chemicals, rust, or oil. Contaminated condensate can affect the quality of water in the system, posing a risk to the environment or workers' health.

o Corrosion and Material Degradation

▪ Corrosion can weaken steam lines over time, particularly when the system operates in harsh environments or with untreated water. Corroded pipes are more prone to leaks, failures, and ruptures.

▪ Condensate can be corrosive, especially if it contains dissolved oxygen, carbon dioxide, or other contaminants. Corrosion of the condensate lines can lead to leaks or ruptures, potentially releasing hot water or causing system failures.

o Vibration and Fatigue

▪ The flow of high-pressure steam can cause the pipes to vibrate, leading to fatigue and potential failure of the pipe material or joints.

o Inadequate Ventilation

▪ If steam leaks occur in enclosed spaces without adequate ventilation, the rapid temperature and pressure changes can cause damage to both the steam system and the environment. Additionally, in some cases, steam could displace oxygen, creating a suffocation risk.

▪ Condensate lines that run through confined spaces or areas without proper ventilation can pose risks if leaks occur. In poorly ventilated spaces, leaks can lead to steam accumulation or condensation, increasing the risk of burns, scalding, or creating hazardous conditions.

o Human Error

O&M Procedure VA Project # 556-24-108

O&M Procedure April 2025 5

▪ Mistakes during maintenance, operation, or emergency response can lead to serious accidents, including steam leaks, improper handling of high-pressure steam, or failure to follow proper shutdown procedures.

▪ Maintenance or repair work on condensate lines can be dangerous if not performed correctly. For example, workers may accidentally release trapped condensate, causing scalding injuries, or mishandle equipment, leading to leaks or pressure imbalances.

o Temperature Shock/Thermal Shock

▪ Rapid temperature changes, such as when cold water is introduced into a hot steam line, can cause thermal shock. This can result in pipe fractures, steam escapes, or mechanical failures.

o Backflow or Reverse Flow

▪ Condensate can flow back into the steam system if the condensate return system is not designed properly or if there is a failure in the condensate pump or check valves. This can lead to contamination of the steam system, potentially causing corrosion, blockages, or pressure instability.

CAST IRON GATE, GLOBE AND CHECK VALVE

INSTALLATION, OPERATION AND MAINTENANCE INSTRUCTIONS

INSTALLATION

1) Thoroughly clean and prepare the piping system before valve installation.

2) Remove the valve end caps if present and inspect the valve ports and seating surfaces for cleanliness just prior to installation.

3) Support the valve to prevent unnecessary stresses induced by connecting pipe.

4) Be sure the rating of the valve is compatible with the intended service conditions. Recommended aptitude is with the stem in the vertical position. Horizontal stem installations can present issues for larger valves, especially in reduced pressure service such as low pressure(5-35 psi) steam.

5) Bolt flange in line per recommendations of applicable ASME/ANSI standard, e.g., ASME/ANSI B16.1. Use full face gaskets.

6) Operate the valve from the full open to closed position.

7) Verify the tightness of the packing nut/gland after installation.

8) Recommended Gate and Globe Valve installation: Stem in the vertical position with operator on top.

NOTE 1: Check valves may be installed in both horizontal and vertical lines with upward flow or in any intermediary position, as long as the hinge pin on swing checks is horizontal. Make sure they are sufficiently distant from pump outlets or other turbulence including devices. See Technical Bulletin.

NOTE 2: Check valves may be shipped with internal packaging to prevent disc damage in transit.

Remove any packing material prior to installation.

OPERATION

1) Gate and globe valves are manually operated. To open, turn the handle in a counterclockwise direction. To close, turn the handle in a clockwise direction.

2) Check valves are automatic.

INSPECTION & MAINTENANCE

1) Periodic inspection and preventative maintenance is not required other than adjustment of stem packing, and cycling of the valve from open to closed position.

2) If a valve develops a packing leak, adjust the packing gland nut to increase the pressure on the stem packing. The packing nuts should be turned in a clockwise direction approximately 1/4 turn, or until the leakage stops. Do not repack under pressure.

3) Whenever a new stem is installed, the packing should be changed also.

REPAIR PARTS

Under normal conditions, repair parts are not required. Parts that may be considered recommended spare parts are as follows:

Gate Valves-Packing, Bonnet Gasket Globe Valves-Packing, Bonnet Gasket Check Valves-Disc Kit, Hanger Kit, or Repair Kit

When ordering parts provide the figure number of the valve, size, part needed and age of the valve.
