C. Volume I - Technical Capability Technical Capability is a minimum acceptability requirement. A blanket statement that the quoter will comply with the PWS is not sufficient.

The Total Evaluated Price will be the sum of CLINs 0001, 1001, 2001, 3001, 4001, and 5001. CLIN 5001 is included for evaluation purposes in accordance with FAR 52.217-8 and does not obligate the Government to exercise theextension.
7. FAR 52.212-2 - EVALUATION - COMMERCIAL PRODUCTS AND COMMERCIAL SERVICES
FAR 52.212-2 is incorporated by reference with the following fill-in and addendum. The Government intends to award a contract to the responsible small business quoter whose quotation conforms to the RFQ, is technically acceptable, and has the lowest evaluated price.
Evaluation factors are: (1) Technical Capability and (2) Price. Technical Capability will be evaluated on an ACCEPTABLE/UNACCEPTABLE basis. Award will be made using a lowest-priced technically acceptable (LPTA) approach.
ADDENDUM TO FAR 52.212-2 - EVALUATION METHODOLOGY
Factor 1 - Technical Capability Technical Capability will be evaluated as ACCEPTABLE or UNACCEPTABLE. To receive an ACCEPTABLE rating, the quotation must clearly demonstrate, with objective supporting information, that the quoter possesses the personnel, qualifications, manufacturer/OEM access, technical resources, parts capability, staffing, and processes necessary to perform every material PWS requirement for all equipment identified in Table 1.4.
The Government will evaluate the completed Equipment Capability Matrix and supporting documentation. The Government may verify submitted information with manufacturers, references, Government databases, or other reasonably available sources.
A quotation will be rated UNACCEPTABLE if it fails to demonstrate capability for one or more material PWS requirements or equipment categories, or if required supporting documentation is absent or insufficient. An UNACCEPTABLE quotation will not be considered further for award.
Factor 2- Price Price will be evaluated for reasonableness. The Total Evaluated Price is the sum of the Base Period, all four Option Periods, and six months of services under FAR 52.217-8. For evaluation purposes only, the six-month extension will be calculated using the Option Year 4 monthly unit price multiplied by six. Evaluation of the extension does not obligate the Government to exercise FAR 52.217-8. T The Government may compare quoted prices with other quotations, historical prices, independent Government estimates, market research, and other price-analysis techniques permitted by FAR 13.106-3.
Award Decision The Government will first determine whether each quotation is technically ACCEPTABLE or UNACCEPTABLE. Only technically acceptable quotations will be considered for award. Award will be made to the responsible small business quoter submitting the lowest Total Evaluated Price among technically acceptable quotations, provided the price is determined reasonable and the quotation otherwise conforms to the RFQ.
8. OPTIONS
The Government intends to evaluate the Base Period, all four Option Periods, and the potential six-month extension under FAR 52.217-8 for award. FAR 52.217-5, Evaluation of Options; FAR 52.217-8, Option to Extend Services; and FAR 52.217-9, Option to Extend the Term of the Contract, shall be included as applicable in the final contract-writing-system clause set. For evaluation purposes, FAR 52.217-8 will be priced as six months at the Option Year 4 monthly unit price. Evaluation does not obligate the Government to exercise any option.
FAR 52.217-8 - OPTION TO EXTEND SERVICES (NOV 1999)
The Government may require continued performance of any services within the limits and at the rates specified in the contract. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days.
9. CONTRACT TERMS AND CONDITIONS
The resultant contract will be a standalone, firm-fixed-price commercial contract subject to the terms and conditions set forth in this solicitation, the Performance Work Statement (PWS), and all applicable FAR, DFARS, and DHA regulations. The Contracting Officer shall incorporate the applicable clause set before award. FAR 52.212-4, Contract Terms and Conditions — Commercial Products and Commercial Services, and FAR 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders — Commercial Products and Commercial Services, apply as set forth in Attachment 4.
10. QUESTIONS AND COMMUNICATIONS
All questions, inquiries, and requests for clarification regarding this RFQ shall be submitted in writing via email to the Contract Specialist at [insert.email@health.mil] and the Contracting Officer at zachary.martens.civ@health.mil no later than October 13, 2026, 10:00 AM Eastern. The Government may consolidate and publish questions and responses via an official solicitation amendment posted to SAM.gov without identifying the submitting entity. Changes to this RFQ will be made only by written amendment issued by the Contracting Officer. Telephonic questions will not be accepted.
11. AWARD AND ORDER ADMINISTRATION
The Government intends to issue one contract. The successful contractor shall not commence performance before receipt of the executed order or other written direction from the Contracting Officer. Only the Contracting Officer may change the scope, price, terms, or conditions of the order.
