1. INTRODUCTION
1.1. Purpose of the work is to survey and inventory all cross connections throughout the installation, in accordance with this SOW at PSFB.
1.2. The Contractor shall provide all labor, materials, equipment, and resources necessary to complete the 5 Year Backflow Survey at PSFB, as defined in this SOW. All work must comply with applicable federal, state, and local regulations, including Occupational Safety and Health Administration (OSHA), Unified Facilities Criteria (UFC), and manufacturer standards.
2. GENERAL REQUIREMENTS
2.1. The Contractor is responsible to verify site conditions, measurements, quantities, and materials needed to complete this project prior to submitting a proposal.
2.2. Estimated Performance Time: 90- calendar days.
2.3. All work, equipment, and materials required for this contract shall be staged in an area coordinated with the Base Civil Engineer (BCE) personnel or Government designated representative (i.e., Contracting Officer, Project Manager, Construction Manager). This staging area will not interfere with building operations or access to PSFB. Staging area will be cordoned off appropriately per applicable safety requirements.
2.4. All work accomplished by the Contractor shall comply with the contract criteria unless a waiver is obtained from the Air Force when conditions warrant such consideration. All construction work shall be subject to stoppages and delays due to critical PSFB mission requirements.
2.5. The Contractor shall, within ten days after the Notice to Proceed (NTP) (or timeframe determined by the Contracting Officer), submit a practicable electronic work schedule for approval. This schedule should detail the order of work, anticipated start and completion dates for key tasks, and be presented as a progress chart.
2.6. PROJECT SCOPE
2.7. Contractor shall submit a plan on how to survey all facilities and water-using equipment and systems including equipment, sample forms and software 30 days prior to work start date. PSFB includes 182 buildings and approximately 3.8 million square feet of area.
2.8. Contractor shall document the location, degree of hazard, record backflow prevention device model number and serial number data, hazard description and recommended appropriate device for hazard.
2.9. Contractor shall document newfound hazards and notify 21st Civil Engineer (21 CES) representative the week of discovery.
2.10. Upon completion of field work, contractor shall submit within 30 days a draft report of findings of discrepancies and recommended corrective actions. Upon government review contractor shall incorporate changes and submit final reports for approval within 30 days.
2.11. Survey shall be done in accordance with DAFMAN 32-1067 section 3.3.4.8, UFC 3-600-
3. UTILITY OUTAGES
3.1. Any required outages (i.e., electrical, HVAC, utilities, fire alarm, fire suppression, etc.) shall be formally requested through Customer Service. Fire alarm outages should be requested 3 days ahead, while all other outages shall be requested 14 days in advance.
4. ENVIRONMENTAL AND HAZARDOUS MATERIAL HANDLING
4.1. The Contractor shall follow all local, state, and federal laws pertaining to the survey work. All environmental hazards found or identified at PSFB while executing this work will immediately be reported to 21ces_ceie.distro@us.af.mil or Bob Tomlinson at 719-556-6100 in 21 CES Environmental Section at 719-556-8412.
5. CONTRACTOR QUALIFICATIONS AND EXPERIENCE
5.1. The contractor shall have at least 5 years’ experience in backflow and cross-connection survey work. Contractor shall have completed at least two similar surveys in the past. Documentation of experience and past work must be submitted as part of the contract proposal.
6. WORK SCHEDULE
6.1. The Contractor shall perform services required under this SOW during the hours of 0700 thru 1600 hours, Monday through Friday. Requests to work beyond or outside of these hours shall be submitted to Contracting Officer for approval. The contractor will not be able to work on federally recognized holidays or special events. If there is a special event that comes up during the hours of 0800 thru 1630, the contractor will be notified as soon as possible but the contractor should be prepared to stop work on short notice.

7. WORK AREA ACCESS
7.1. This survey is located across the installation including restricted areas and all contractors must have proper authorization. Facility access will require advanced coordination with facility managers.
Upon acceptance of this contract, the contractor shall complete and submit a PSFB Visitor Request to 21st Contracting Squadron. All PSFB Visitor Request requests shall be submitted no later than 15 business days prior to the first day of work. The Contractor shall coordinate EAL processing with the Contracting Officer. Any contractor who fails to pass a background check will not be permitted to work on the installation. Note that cell phones, computer tablets and similar devices may not be allowed in restricted area facilities.
8. PROTECTION OF GOVERNMENT PROPERTY
8.1. The Contractor shall use reasonable care to avoid damaging existing buildings, equipment, and vegetation on the government installation. If the Contractor fails to use reasonable care and causes damage to any of this property, the Contractor shall replace or repair the damage at no expense to the Government. If the Contractor fails or refuses to make such repair or replacement, the Contractor shall be liable for the cost, which may be deducted from the contract price.
9. SITE MAINTENANCE AND CLEANUP
9.1. The Contractor shall maintain a clean and organized, and safe worksite, protecting all areas from debris. Work areas shall be clean daily, with waste place in designated contained and removed from government property according to regulation. Upon completion, the site shall be restored to a clean and orderly condition, acceptable to 21 CES.
10. DELIVERABLES
10.1. Submit all equipment specifications prior to ordering any parts or equipment for 21 CES approval.
10.2. Upon completion of field work, contractor shall submit within 60 days a draft report of findings of discrepancies and recommended corrective actions. Upon government review contractor shall incorporate changes and submit final reports for approval within 30 days.
10.3. Contractor will provide an electronic copy and 2 hard copies of reviewed and approved report.
11. INSPECTION AND ACCEPTANCE
11.1 All work and deliverables specified within this SOW shall be subject to inspection and acceptance by the Government. The Contracting Officer is the only individual with the authority to formally accept or reject work on behalf of the Government, based on the technical recommendation 21 CES.
11.2. The Government will inspect all deliverables to ensure they meet the requirements of this SOW. Deliverables shall be prepared in a professional manner, free of technical and grammatical errors. The standard for acceptance shall be full compliance with the requirements set forth in this SOW.
