About this file
This is a sworn statement form required by Kentucky Revised Statute 45A.395 for the City of Owensboro, Kentucky in connection with a soft body armor procurement for the Owensboro Police Department. The document serves as a prerequisite certification that bidders and offerors must submit to be deemed responsible and eligible for contract award. Bidders must certify under penalty of perjury that neither they nor anyone acting on their behalf has knowingly violated any provision of Kentucky's campaign finance laws and that awarding a contract to them will not violate such laws.
The form requires completion of standard information fields including the bidder's company name, authorized representative's signature, title, office, and the date of execution. The statute defines "knowingly" as awareness or should-be awareness that conduct violates campaign finance law or that a circumstance exists as described in the statute. This certification is a mandatory compliance document that protects the City of Owensboro by ensuring bidder integrity and adherence to state campaign finance regulations prior to contract execution.
