About this file

This is a Vendor's Statement of Compliance form required by the City of Owensboro, Kentucky, pursuant to KRS 45A.343, for vendors seeking to contract with the municipality. The statement requires vendors to certify that they have not been found in violation of specified Kentucky Revised Statutes (KRS) chapters related to corporate taxes, sales and use taxes, income taxes, wage and hour regulations, occupational safety, unemployment insurance, and workers' compensation within the five-year period preceding the statement. The form serves as a compliance attestation for the Owensboro Police Department's soft body armor procurement, with vendors required to acknowledge their commitment to maintaining compliance with applicable statutes throughout the contract duration.

The statement establishes consequences for nondisclosure or noncompliance, permitting the City of Owensboro to cancel contracts with vendors found in violation and to disqualify such vendors from eligibility for future city contracts for a period of two years. The form requires completion and signature by a duly authorized officer of the vendor company, including the company name, authorized representative signature, title, and date of execution. This compliance certification is a mandatory prerequisite for vendor participation in the City of Owensboro's procurement process and applies to all applicable provisions of the specified KRS chapters based on the nature of the vendor's business operations.
