About this file

The documents are a Form W-9 (Request for Taxpayer Identification Number and Certification) from the Department of the Treasury Internal Revenue Service, and a Conflict of Interest Questionnaire (Form CIQ) from the Texas Ethics Commission. The W-9 form is a standard tax document used to collect taxpayer information for reporting purposes, with the March 2024 revision highlighting updates to how disregarded entities and flow-through entities complete the form. The Conflict of Interest Questionnaire is designed for vendors doing business with local governmental entities in Texas, reflecting changes made by H.B. 23 from the 84th Legislative Session.

The documents require detailed disclosure of potential conflicts of interest for vendors engaging with local government, including reporting any employment or business relationships with local government officers or their family members. The CIQ must be filed within 7 business days of a vendor becoming aware of facts necessitating the statement, and vendors are required to disclose any taxable income relationships, corporate connections, or gifts given to government officers. Failure to properly complete and submit the form can result in a misdemeanor offense under Section 176.006 of the Local Government Code.

New line 3b has been added to this form. A flow-through entity is required to complete this line to indicate that it has direct or indirect foreign partners, owners, or beneficiaries when it provides the Form W-9 to another flow-through entity in which it has an ownership interest.
