About this file
This is a Certification Regarding Debarment, Suspension, and Other Responsibility Matters form issued by the Incorporated County of Los Alamos Procurement Division in Los Alamos, New Mexico for the Abiquiu Hydroelectric Crane Replacement project. The form requires bidders to certify their eligibility to contract with the county by affirming that neither they nor their principals are currently debarred, suspended, proposed for debarment, or voluntarily excluded from covered transactions by any federal, state, or local agency. Bidders must also confirm that within the preceding three years, they have not been convicted of fraud, criminal offenses related to public transactions, antitrust violations, embezzlement, theft, forgery, bribery, or falsification of records; have not violated contract provisions through deliberate failure to perform or unsatisfactory performance; and have not violated ethical standards established in the County Procurement Code. Additionally, bidders must certify they are not current or former County employees, are not immediate family members of County employees or public officials, and have not had any public contracts terminated for cause or default within the three-year period.
Bidders unable to certify any statement must provide written explanation with their submission. The form requires certification from an authorized representative with signature, date, and title, and includes a contractor confirmation that all proposed subcontractors are not currently suspended or debarred from conducting business with any city, state, county, or federal government entity. Falsification of any statement on the form constitutes grounds for non-consideration of the proposal or rescission of a contract award. The form version effective June 30, 2025, indicates this certification requirement is standard for County of Los Alamos procurement transactions and serves as a prerequisite for bid eligibility and contract award consideration.
