About this file

This document is a Notice & Certification of Contractor of Valid Criminal Records Summary for a school district, addressing requirements under California Education Code Section 45125.1. The document mandates that any entity contracting with a local education agency must ensure employees who interact with students outside of immediate supervision have a valid criminal records summary obtained through Department of Justice (DOJ) fingerprint checks. Contractors must verify that none of their employees who may come into contact with students have been convicted of violent or serious felonies as defined in specific Penal Code sections.

The certification requires contractors to personally review criminal records summaries of employees performing district services and declare under penalty of perjury that no employees have disqualifying criminal convictions. The document provides a formal template for contractors to complete, including spaces to list employees, execute the certification, and provide contact information. Contractors must continuously update the certification if new employees are assigned to district work, ensuring ongoing compliance with student safety requirements through comprehensive criminal background screening.
