About this file

This is a Minority/Woman/Small Business Enterprise (M/W/SBE) Utilization Form required by the City for the Pressure Washing and Exterior Cleaning Services contract opportunity in North Carolina. The form serves as a mandatory submission component of proposal packages and documents the bidding company's certification status and commitment to including M/W/SBE subcontractors in the project. Bidders must identify whether their company holds City certification as a Minority-owned Business Enterprise (MBE), Woman-owned Business Enterprise (WBE), or Small Business Enterprise (SBE), and must provide the certifying agency name along with the effective and expiration dates of such certification. The form requires detailed documentation of outreach efforts undertaken to identify and maximize M/W/SBE participation both during the proposal phase and throughout the contract performance period.

The form mandates submission of a complete list of all M/W/SBE subcontractors intended for the project, including descriptions of work or materials to be provided, appropriate M/W/SBE designations, City vendor numbers, and calculated utilization percentages for each category and combined totals. The City emphasizes a strong commitment to M/W/SBE inclusion when viable subcontracting opportunities exist and recommends that bidders exhaust all available efforts to identify potential M/W/SBE participants. A company representative must sign and date the completed form, certifying the accuracy of the information provided.
