About this file

This is a Past Performance Questionnaire designed by the Department of Homeland Security to evaluate contractor performance for potential Department-wide requirements involving commercial Cloud Service Provider (CSP) Anything as a Service (XaaS) cloud services, professional services, marketplace solutions, and training. The questionnaire covers Infrastructure as a Service (IaaS), Platform as a Service (PaaS), Software as a Service (SaaS), and emerging service models including Functions as a Service (FaaS), Database as a Service (DBaaS), and Container as a Service (CaaS). The form is specifically referenced as "Attachment 4" and relates to solicitation for Public Safety Telecommunicator and Alarm Monitoring Support Services. Completed responses should be emailed to Gregory Blaszko at Gregory.Blaszko@hq.dhs.gov.

The questionnaire requests information from individuals responsible for administrative oversight (project managers, Contracting Officer Representatives, or equivalent personnel) regarding a contractor's past performance on a completed or ongoing contract. Respondents must provide contract number or project title, period of performance including option periods, total project value including options, annual hours, and project scope description. The evaluation covers nine performance areas: consistency in providing required staffing and services; maintenance of records and certifications; professionalism and workforce bearing; problem identification and resolution; responsiveness to customer issues; quality control; overall project management; transition and phase-in capability; and supervision adequacy. Additional questions address subcontracting plan compliance and good faith efforts toward subcontracting goals. The final assessment requires respondents to rate overall contractor performance as Highly Acceptable, Acceptable, or Unacceptable, with corresponding risk characterizations. Respondent contact information and position details must be provided.
