Past Performance Form Summary

This is a standardized federal contracting template (Attachment 3) that contractors must complete when responding to solicitations to document their relevant past performance on similar government or commercial contracts. The form requires contractors to provide comprehensive information about a single previous contract, including the contractor name, customer agency or organization name and complete address, contract number, project award date, total project price including all options, current annual estimated price, period of performance, security clearance requirements if applicable, annual level of effort in hours, number of employees utilized, and geographic locations where work was performed.

The form mandates identification of two customer points of contact: a technical point of contact (program/project manager, quality assurance representative, or technical representative familiar with work quality) and a contracting/purchasing point of contact (contracting officer, purchasing agent, or procurement representative familiar with contractor performance). Contractors must verify whether a finalized Contractor Performance Assessment Report (CPAR) is resident in SAM.gov for the contract period and check the appropriate box accordingly. The critical narrative section (Item 14) requires contractors to describe the scope, magnitude, and complexity of the past contract work, compare and contrast it to the current solicitation requirements, identify any unique or unusual contract requirements, document any performance problems or Department of Labor investigations for labor violations, and explain corrective actions taken. Item 15 addresses subcontracting, requiring disclosure of any subcontractors used, subcontracting plan submission, and the extent of work performed by subcontractors.
