About this file

This is a Vendor Disclosure Statement form required by the Colorado Department of Corrections for prospective vendors seeking state contracts. The form applies to Solicitation Number DQ 2026 and requires vendors to disclose the geographic locations where contract services, including any subcontract work, will be performed. Specifically, vendors must identify all counties and states where services will be delivered. If any portion of the work is anticipated to occur outside the United States or the State of Colorado, vendors must provide a detailed explanation of the necessity or business advantage for performing services outside these geographic boundaries.

The disclosure statement is mandatory for all prospective vendors prior to contracting or during the solicitation process, with specific exceptions for contracts under Medicare, the Colorado Medical Assistance Act, the Children Basic Health Plan Act, and the Colorado Indigent Care Program. Construction contracts are also exempt from this disclosure requirement. Vendors must complete the form by providing their company name, identifying all service locations, and signing and dating the statement with their printed name and title. This disclosure mechanism serves as a pre-award compliance tool to ensure transparency regarding where contracted services will be performed and to establish clear justification for any work performed outside traditional state and national boundaries.
