About this file
This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections (CDOC) for solicitation number DQ 2027000092 as a non-construction purchase order. The form is a mandatory document that must be completed and returned with the bid submission. The form requires vendors to disclose whether any services under the contract or subcontracts will be performed outside the United States or the State of Colorado. If services are to be performed outside these jurisdictions, vendors must identify the type of services being performed, specify the countries or states where work will occur, and provide justification for why performing services outside the U.S. or Colorado is necessary or advantageous. The form must be signed by the contractor's President, Board Chairperson, or authorized signatory.
The procurement office will review the vendor's response to determine whether notice of out-of-state or international performance was provided within the required 20-day timeframe as mandated by Colorado Revised Statutes 24-102-205 and 24-102-206. The form is designed to ensure compliance with Colorado state law regarding public notice of contract performance outside specified geographic boundaries. All responses must be returned to the sole contact listed in the solicitation document.
