About this file
This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado in accordance with Colorado Revised Statutes 24-102-206. The form requires vendors bidding on state contracts to disclose whether any services or subcontracted work will be performed outside the United States or the State of Colorado. Bidders must complete the form by identifying the type of services to be performed externally, specifying the countries or states where performance will occur, and explaining the necessity or business advantage of performing work outside Colorado or the United States. The form must be signed by an authorized company representative, including the business president, board chairperson, or contract authorized signatory, and returned with the bid submission to the sole contact designated in the solicitation.
The form applies to the Boiler Chemicals & Treatment Services contract (Solicitation Number DQ 2027000071) and is structured as a yes-or-no disclosure requirement. Vendors answering "no" to external performance need only provide a signature, while those answering "yes" must complete all supplementary questions. The procurement office will review whether vendors provided required notice within 20 days of contracting or deciding to contract outside the United States or Colorado. This disclosure mechanism ensures state procurement transparency regarding offshore or out-of-state service delivery arrangements.
