About this file

This is a vendor disclosure and notification form required by the Colorado Department of Corrections (CDOC) for State of Colorado contracts involving services performed outside the United States or Colorado. The form mandates that bidders disclose whether any contract services or subcontracted work will be performed outside U.S. or Colorado boundaries. Vendors must complete the form and return it with their bid submission to the sole contact listed in the solicitation. The form requires vendors to answer whether services will be performed outside the specified geographic areas, and if answering affirmatively, to describe the type of services, identify the specific countries or states where services will be performed, and explain the necessity or advantage of performing work outside the United States or Colorado.

The form applies to non-construction contracts and is completed by an authorized business representative such as the President, Board Chairperson, or Contract's Authorized Signatory. The document includes signature, printed name, title, and date fields for vendor authorization. The procurement section of the form requires documentation of the solicitation number, contract type, services performed outside the U.S. or Colorado, and confirmation of whether the vendor provided notice within 20 days of contracting or deciding to contract outside the specified geographic areas. This disclosure requirement aligns with Colorado Revised Statutes 24-102-205 and 24-102-206, establishing transparency regarding offshore or out-of-state service performance in state contracts.
