About this file

This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections for non-construction contract performance. The form is mandated under Colorado Revised Statutes 24-102-206 and serves as notice to the Department of Personnel when a vendor or subcontractor intends to perform services outside the United States or the State of Colorado. Vendors bidding on solicitation number DQ 2027000048 must complete and return this document with their bid submission. The form requires contractors to disclose whether any services will be performed outside the U.S. or Colorado, identify the specific types of services (such as software, hosting, technical support, server monitoring, or design and development), specify the countries or states where services will be performed, and explain the necessity or advantage of performing work outside Colorado or the United States.

The form must be completed and signed by the contractor's President, Board Chairperson, or Authorized Signatory and submitted to the sole contact listed in the solicitation. For procurement purposes, the form documents the type of contract (identified as a Purchase Order for this solicitation) and whether the vendor provided required notice within 20 days of deciding to contract outside the U.S. or Colorado. This disclosure requirement applies to all vendors regardless of whether they answer affirmatively or negatively to the question of outside performance, with vendors answering "No" required only to sign and return the form.
