About this file

This is a vendor disclosure and notification form required by the Colorado Department of Corrections (CDOC) in accordance with Colorado Revised Statutes 24-102-206, designed to capture information regarding contract performance conducted outside the United States or the State of Colorado. The form must be completed and submitted by bidders as part of their bid response and is applicable to non-construction service contracts. The document requires vendors to disclose whether any services under the primary contract or any subcontracts will be performed outside the United States or Colorado, and if so, to provide detailed information regarding the type of services, specific locations where services will be performed, and justification for performing work outside these jurisdictions.

The form is directed to business presidents, board chairpersons, or contract-authorized signatories and requires their signature, printed name, title, and date upon completion. For procurement use, the form captures the solicitation number, contract type, nature of services performed outside the U.S. or Colorado, and verification of whether the vendor provided notice within 20 days of contracting or deciding to contract outside the specified jurisdictions. This disclosure mechanism serves as formal notification to the Department of Personnel regarding vendor commitments to perform work outside domestic and state boundaries, ensuring transparency and compliance with Colorado statutory requirements governing offshore or out-of-state contract performance.
