About this file

This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections (CDOC) for the State of Colorado regarding contract performance outside the United States or Colorado. The form serves as a mandatory disclosure document that must be completed and returned with all bids in response to the Fresh Eggs Request for Quote. The form requires bidders to declare whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, to identify the specific services, locations, and justification for performing work outside these jurisdictions. Bidders must provide contractor name, authorized signatory information, and submit the form to the sole contact listed in the solicitation.

The form establishes compliance requirements under Colorado Revised Statutes 24-102-206 and 24-102-205, requiring vendors to provide written notice if work will be performed internationally or outside Colorado. Vendors must disclose the type of services to be performed outside the U.S. or Colorado (such as software, hosting, technical support, or design and development), specify the countries or states where work will occur, and explain the necessity or business advantage of performing services outside the specified jurisdictions. The form includes sections for procurement use only to document the solicitation number, contract type, services performed outside the U.S. or Colorado, and whether the vendor provided notice within 20 days of contracting or deciding to contract outside the United States or Colorado.
