About this file

This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, required to be completed and submitted with bids for a Fresh Produce Request for Quote. The form serves as notice pursuant to Colorado Revised Statutes 24-102-206 that a vendor or its subcontractor may perform services outside the United States or the State of Colorado. Bidders must answer whether any services under the contract or subcontracts will be performed outside these jurisdictions and, if so, provide detailed information about the nature of services, locations where services will be performed (including specific countries and states), and justification for performing work outside the specified geographic areas.

The form requires completion and signature by an authorized representative such as the business president, board chairperson, or contract authorized signatory. Vendors must disclose whether they intend to perform services outside the U.S. or Colorado, describe the specific services to be performed, identify all countries or states where work will occur, and explain the necessity or advantage of performing services outside the prescribed jurisdictions. The procurement office is responsible for tracking whether vendors provided timely notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado. This disclosure mechanism ensures compliance with Colorado state law regarding offshore or out-of-state contract performance.
