About this file

This is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for non-construction contracts that may involve services performed outside the United States or the State of Colorado. The form serves as notice to the Department of Personnel regarding any contract awards where vendors or their subcontractors intend to perform work outside these geographic boundaries in accordance with Colorado Revised Statutes 24-102-206. Bidders must complete and return this document with their bids, with the authorized signatory being the business president, board chairperson, or contract's authorized signatory. The form requires vendors to answer whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, to provide details regarding the type of services, locations where services will be performed, and justification for performing work outside these geographic areas.

The form includes a section for procurement use only that documents the solicitation number, contract type, services performed outside the U.S. or Colorado, and whether the vendor provided notice within 20 days of contracting or deciding to contract outside the U.S. or Colorado, as required by statute. This disclosure requirement applies to contracts related to the Fresh Produce Request for Quote solicitation in Colorado and ensures transparency and compliance with state law regarding offshore or out-of-state service delivery arrangements.
