About this file
This document is a vendor disclosure and notification form issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, specifically designed for non-construction contracts requiring performance outside the United States or Colorado. The form must be completed and returned with bids in response to a Fresh Eggs Request for Quote. Bidders are required to disclose whether any services under the contract or subcontracts will be performed outside the United States or the State of Colorado, and if so, must identify the specific services, geographic locations, and justification for performing work outside these jurisdictions.
The form serves as formal notice to the Colorado Department of Personnel regarding vendor intent to perform contract services outside the United States or Colorado, in compliance with Colorado Revised Statutes 24-102-206. Vendors must provide written notification within 20 days of contracting or deciding to contract outside the US or Colorado. The form includes signature blocks for authorized company representatives (President, Board Chairperson, or Contract's Authorized Signatory) and sections for procurement office use only to document the solicitation number, contract type, services location, and compliance with the 20-day notification requirement.
