About this file
This is a vendor disclosure and notification form required by the Colorado Department of Corrections for non-construction contracts performed outside the United States or Colorado, in accordance with Colorado Revised Statutes 24-102-206. The form is mandatory and must be completed and returned with the bid submission by the contractor's President, Board Chairperson, or authorized signatory. Bidders must disclose whether any services under the contract or subcontracts will be performed outside the United States or Colorado, and if so, provide specific details regarding the type of services, locations where services will be performed (by country and/or state), and justification for performing work outside these jurisdictions.
The form requires vendors to answer a primary question regarding out-of-state or out-of-country service performance, with conditional completion requirements based on the response. If services will be performed externally, vendors must complete three additional sections describing the service types, geographic locations, and business rationale. The form includes a signature section for the authorized contractor representative and a procurement-use-only section for agency tracking, including documentation of whether the vendor provided required notice within 20 days of contracting or deciding to contract outside the United States or Colorado.
