About this file
This is a Vendor Disclosure and Notification form (Exhibit E) issued by the Colorado Department of Corrections (CDOC) for the State of Colorado, designed to capture information regarding contract performance conducted outside the United States or the State of Colorado. The form requires bidders to disclose whether any services under the contract or subcontracts will be performed outside these jurisdictions. Bidders must answer a yes or no question regarding anticipated offshore or out-of-state service performance, and if answering affirmatively, provide detailed descriptions of the services to be performed, the specific countries or states where performance will occur, and a justification for why such offshore or out-of-state performance is necessary or advantageous.
The form must be completed and returned with the bid submission to the sole contact listed in the solicitation. All responses require authorization by the contractor's President, Board Chairperson, or the Contract's Authorized Signatory. The form is governed by Colorado Revised Statutes 24-102-205 and 24-102-206, which establish the statutory requirements for vendor notification regarding performance of services outside the United States or Colorado. Upon completion, procurement staff will review the submission to verify whether the vendor provided notice within 20 days of contracting or deciding to contract outside the US or Colorado, as required by statute.
