About this file

This document is a Vendor Disclosure & Notification form (Exhibit E) issued by the Colorado Department of Corrections for the State of Colorado in accordance with Colorado Revised Statutes 24-102-206. The form serves as a disclosure mechanism requiring vendors to notify the Department of Personnel when contract performance or subcontract services will be performed outside the United States or the State of Colorado. Bidders must complete and return this form with their proposals, with completion authorized by the business President, Board Chairperson, or Contract's Authorized Signatory. The form requires vendors to answer whether any services will be performed outside the U.S. or Colorado, and if affirmative, to describe the type of services, specify the geographic location(s) of performance, and explain the necessity or advantage of performing services outside these jurisdictions.

The form contains no pricing information, set-asides, or funding details, as it functions solely as a compliance and disclosure document. For procurement use, the form captures the solicitation number, contract type, services performed outside the U.S. or Colorado, and whether the vendor provided required notice within 20 days of contracting or deciding to contract outside the specified jurisdictions. This disclosure requirement enables the state to track and monitor vendor performance locations for accountability and compliance purposes under Colorado procurement law.
