About this file

This is a vendor disclosure and notification form (Exhibit E) issued by the Colorado Department of Corrections for a non-construction contract related to a Fresh Eggs Request for Quote (Solicitation Number DQ 2027000114). The form serves as required notice under Colorado Revised Statutes 24-102-206 and must be completed and returned by bidders as part of their bid submission. The document requires vendors to disclose whether any services under the contract or subcontracts will be performed outside the United States or the State of Colorado, and if so, to identify the specific services, locations, and business justification for such out-of-state or out-of-country performance.

The form mandates that the authorized signatory (business president, board chairperson, or contract authorized signatory) complete four sections: confirmation of whether services will be performed outside the U.S. or Colorado, description of the service types, identification of geographic locations where services will be performed, and explanation of the necessity or advantage of performing services outside the specified jurisdictions. Additionally, the procurement office must verify whether the vendor provided notice of out-of-state or out-of-country performance within 20 days of contracting or deciding to contract outside the U.S. or Colorado, as required by statute. No pricing information, award amounts, contract terms, or other financial details are specified in this disclosure form.
