About this file
This is a Bidder's Disclosures and Certifications form required for the City of Alpharetta's RFP 26-115, On-Call Pipe Lining Services procurement. The City is seeking proposals from contractors for trenchless stormwater systems rehabilitation, with plans to enter into on-call contracts with up to two contractors—one for cured-in-place pipe lining (CIPP) and one for spincast pipe lining—for an initial period of one year with four optional one-year renewals. Offerors must submit a single proposal indicating whether they can provide CIPP services, spincast services, or both. The City has identified over 500 pipes requiring repair, with additional pipes added annually based on inspection results. No minimum work guarantee is implied under these contracts, as the City determines its capital budget and prioritizes work plans annually based on citywide needs and demands.
Bidders must complete and submit the disclosure and certification form, which requires disclosure of any conflicts of interest or relationships with City elected officials or employees, including financial, personal, family relationships, or campaign contributions made within the past two years. The form also requires certification regarding debarment, suspension, proposed debarment, or ineligibility status; any convictions or civil judgments within the preceding three years related to fraud, criminal offenses on public contracts, antitrust violations, or crimes such as embezzlement, theft, bribery, or falsification of records; current criminal or civil charges; and any contract terminations for default within the three-year period. Bidders must certify non-collusion, compliance with state and federal equal opportunity and non-discrimination requirements, and adherence to Georgia Code Sections 45-10-20 et seq., which prohibit City employees from transacting business with the City in which they are employed or hold a substantial interest. Failure to disclose or submit the completed notarized form may result in the response being declared non-responsive and ineligible for further consideration.
