About this file
This is a cost proposal attachment form for a Secondary Mathematics Curriculum and Textbook Adoption (Grades 6-8) issued by Cache County School District (CCSD) in Utah under RFP #CCSD-10526. The district seeks to adopt comprehensive, research-based mathematics materials for its four middle schools serving approximately 4,559 students in grades 6-8 annually across 36 math teachers (approximately 12 per grade level). The procurement requires non-consumable, reusable student textbooks with 1,620 total textbooks needed (12 classroom sets of 45 textbooks per grade level), digital student access licenses covering all 4,559 students annually, digital and print teacher materials for 36 teachers, and professional learning support consisting of two half-day in-person sessions scheduled for spring 2026 and August 2026. All pricing submissions must reflect a five-year contract term, with student licenses required to be transferable between grade levels to accommodate enrollment fluctuations.
Offerors must provide itemized pricing for digital student access licenses, non-consumable print textbooks, teacher digital access, print teacher materials, professional learning sessions, and shipping costs in the cost proposal form. Pricing components may be bundled, and vendors must clearly indicate included services and any volume-based discounts applied. The district explicitly states it will not purchase consumable workbooks and reserves the right to procure additional student licenses and materials at the same unit pricing established in the awarded contract to accommodate enrollment growth. Cost evaluation will utilize a competitive formula where the lowest-priced proposal receives 100 percent of cost points, with all other proposals receiving a proportional percentage based on their variance from the lowest price; proposals exceeding double the lowest price receive no cost points.

ATTACHMENT D
COST PROPOSAL FORM
RFP #CCSD-10526
Cache County School District has approximately 4,559 students in grades 6–8 per year. Student enrollment may fluctuate annually. CCSD has approximately 36 math teachers in grades 6-8, with approximately 12 teachers at each grade level.
The District does not intend to purchase consumable student workbooks. All student print materials (textbooks) must be non-consumable and reusable. CCSD will purchase classroom sets of approximately forty-five (45) non-consumable student print textbooks for each teacher and grade level taught. For pricing purposes, this equals twelve (12) classroom sets per grade level and a total of one thousand six hundred twenty (1,620) non-consumable student print textbooks across grades 6–8.
Offerors must complete all sections below. All pricing shall reflect the total cost to CCSD for a five (5) year term.
CCSD will evaluate cost using the Total Cost of All Required Components identified in Section E. Optional items not required by this RFP will not be included in the cost evaluation. If pricing components are bundled, offerors shall clearly indicate “Included” and provide a brief description of what is included. Student licenses must be transferable between grade levels to accommodate changes in enrollment. Any additional student licenses, digital access, or non-consumable student print materials required due to enrollment growth shall be made available at the same unit pricing and terms established in the awarded contract.

The offeror with the lowest Proposed Price will receive 100% of the price points. All other offerors will receive a portion of the total cost points based on what percentage higher their Proposed Price is than the Lowest Proposed Price. An offeror whose Proposed Price is more than double (200%) the Lowest Proposed Price will receive no points.
