About this file

This document is a Certification Regarding Debarment, Suspension, Ineligibility and Voluntary Exclusion form required by the U.S. Department of Agriculture for a Bread and Bread Products bid for 2025-2026. The certification is mandatory for any contract equaling or exceeding $25,000 or any contract for audit services, and requires bidders to confirm they are not currently debarred, suspended, ineligible, or voluntarily excluded from participating in federal transactions. The form must be completed and submitted with the bid package for the bread products procurement.

The document outlines specific requirements for bidders, including an obligation to provide immediate written notice if their certification status changes and a commitment to not knowingly enter into transactions with debarred or suspended entities. Participants must include this certification clause in all lower-tier covered transactions and solicitations without modification. The certification serves as a material representation of fact, and knowingly providing false information may result in additional remedies, including potential suspension or debarment by the federal department or agency originating the transaction.
