About this file

This document outlines the Sexual Assault Forensic Examination Payment Program administered by the Utah Office for Victims of Crime (UOVC), in conjunction with the Department of Public Safety's Invitation for Bid (IFB) for BFS Sexual Assault Evidence Collection Kits (AK26-20). The program provides reimbursement for sexual assault examinations that include evidence collection and standard medical care provided to victims during the examination process. To qualify for payment, the sexual assault exam must be reported to law enforcement, and the attending physician or nurse must complete a Fast Track Reimbursement Form and submit it directly to UOVC along with the hospital or medical facility billing invoice. Importantly, sexual assault charges do not need to be proven or substantiated for reimbursement eligibility, and victims shall not be personally charged for the forensic examinations. The IFB seeks to award a single contract to the lowest cost responsive and responsible vendor for a five-year term, with pricing guaranteed for at least one year.

The Fast Track Reimbursement Form must include a law enforcement case number or be signed by a law enforcement officer, victim/witness coordinator, or medical provider. All collateral sources of payment, including local law enforcement reimbursement, health insurance, and Medicaid, must be considered before UOVC processes reimbursement requests, though exceptions may be made. Hospitals and medical facilities must submit the Fast Track Reimbursement Form or billing invoice to UOVC within one year of the sexual assault examination. The program covers only evidence collection costs and standard medical care provided during the exam process; additional medical treatment resulting from the crime, such as sutures, x-rays, follow-up exams, or mental health counseling, requires a separate long-form application for consideration under UOVC's compensation program. For inquiries, the Utah Office for Victims of Crime can be contacted at 350 East 500 South, Suite 200, Salt Lake City, Utah 84111, (801) 238-2360, or toll-free at (800) 621-7444.

Sexual Assault Forensic Examination Payment Program

The Utah Office for Victims of Crime (UOVC) may pay for the cost of sexual assault examinations that provide for the collection of evidence and the standard medical care given to sexual assault victims during the exam process.

Eligibility requirements for the sexual assault examination payment program differ from UOVC’s compensation program. The victim does not need to file a long form application; however, the attending physician or nurse must complete a Fast Track Reimbursement Form and must submit the completed form directly to UOVC along with the hospital or medical facility billing invoice. The Fast Track Reimbursement Form developed specifically for sexual assault exams will expedite the payment of the sexual assault exam and prevent the direct billing of the victim.

Conditions to consider when applying for reimbursement through the sexual assault forensic examination payment program are:

The sexual assault exam must be reported to law enforcement.

The Fast Track Reimbursement Form must include the law enforcement case number or be signed by a law enforcement officer, a victim/witness coordinator, or the medical provider.

Sexual assault charges do not have to be proven or substantiated.

All collateral sources such as local law enforcement reimbursement, health insurance, Medicaid, etc. must be considered before submitting the request to UOVC for payment through this program (some exceptions can be made).

The hospital or medical facility must submit the Fast Track Reimbursement Form or billing invoice to UOVC within one year of the sexual assault exam.

Victims shall not be personally charged for sexual assault exams.

PLEASE NOTE: The sexual assault forensic examination payment program can only pay for the cost of collecting sexual assault evidence and the standard medical care given to victims during the exam process. If, as a result of the crime, the victim requires additional medical treatment such as sutures, x-rays, follow-up exams for injuries and/or mental health counseling, a UOVC long form application must be completed by the victim/claimant for consideration of reimbursement through UOVC’s compensation program. Adult patients are not required to be interviewed by law enforcement to receive a forensic exam or to have the exam paid for by UOVC. Please list any additional crime-related injuries on the Fast Track Reimbursement Form.
