About this file
This is a URS Post-Retired Verification Form for independent contractors (1099 employees) associated with Jordan School District in Utah. The form requires contractors, owners, partners, or principals to disclose whether they have retired from Utah Retirement Systems (URS) and to provide their birthdate and type of business if applicable. The form must be submitted to the Accounting Department along with a W-9 form. Any individual who has received monthly benefits from URS and is returning to work as an employee or independent contractor with a participating URS employer is subject to re-employment restrictions under Utah Code Section 49-11-1207(2).
Retirees receiving URS benefits are required to complete and file a Post-Retirement Employment form (Form RTRT-27A) with URS's Retirement Benefits Department before beginning work with Jordan School District, which can be reached at 801-366-7770. Failure to report re-employment to URS constitutes a violation of Title 49 and may result in the retiree being held liable for repayment of any URS overpayments resulting from non-disclosure. This verification requirement applies to all contractors, regardless of employment classification, and is intended to ensure compliance with state retirement system regulations.
