About this file

This file is a Seed Analysis Request Form instruction document and template for the Utah Division of Wildlife Resources (UDWR) Seed Procurement (AS26-65 UDWR Seeds, Summer 2026). The form enables vendors to submit seed testing information to approved seed laboratories and samplers for analysis prior to delivery to the Great Basin Research Center Seed Warehouse in Ephraim, Utah. Vendors must complete and submit the Analysis Request Form via email to dwrseed@utah.gov at least two business days before seed arrival at the warehouse, with form receipt constituting authorization for testing and payment. The form requires vendors to fill only designated yellow columns with complete seed lot information including vendor name, species common and scientific names, variety or germplasm ID, eco-region and seed zone, lot number, total pounds, number of bags, bag size, certification status, and seed treatment details. Testing options available to vendors include purity, tetrazolium (TZ), noxious weed analysis for all Western States, rush testing, and high confidence (two sample) testing for specified species including various rabbitbrush, kochia, and sagebrush varieties. The State reserves the right to request additional testing such as high confidence testing or downy brome analysis beyond the vendor's initial request, with notification provided via email.

Approved seed laboratories include official state laboratories in Arizona, California, Colorado, Idaho, Indiana, Montana, North Dakota, Oregon, South Dakota, Utah, Washington, and Wyoming, as well as private laboratories including Precision Seed Testing (Applewood Seed Co.) and Agri Seed Testing Inc., plus any USDA approved accredited seed entity with appropriate accreditation scope. All seed test results must be sent to dwrseed@utah.gov, with billing directed to the Utah Division of Wildlife Resources at the GBRC Seed Warehouse (494 W. 100 S. Ephraim, UT 84627; phone 435-283-4441). Vendors should note that if Amaranth species are detected, genetic testing for Palmer amaranth (A. palmeri) verification will likely be required, necessitating prior contact with the Division at dwrseed@utah.gov before initiating such testing. The form includes provisions for additional seed lots through row additions and space for other remarks relevant to the seed analysis request.
