About this file

This document is Attachment D, a Notice and Certification form from Green Bay Area Public Schools (GBAPS) for Request for Proposal (RFP) Q2526-02. The specific associated opportunity involves E-Rate Category 2 licenses for network equipment, where the district is seeking software licenses for network equipment already owned. Vendors are invited to propose licenses for the specified makes/models or equivalent equipment, with the district retaining the right to determine functional and technical equivalence of alternative solutions.

The document outlines key procurement policies, including public records access, proprietary information handling, and submission requirements. Vendors must certify understanding of terms, submit proposals voluntarily, and affirm no collusive practices. The procurement process emphasizes transparency, with provisions for confidential information submission using specific state forms. Bid/proposal prices cannot be held confidential, and all submitted data, documentation, and innovations become property of GBAPS. Vendors must be prepared to defend any proprietary information claims and understand that the school board will ultimately select the successful vendor after an evaluation committee's recommendation.
