About this file

This is a Cost Proposal attachment for a Crisis Communication contract opportunity issued by the State of Utah Division of Risk Management through the State of Utah Division of Purchasing. The RFP seeks a single vendor to provide crisis communication services under a 5-year contract term, with pricing guaranteed for the first year. The contract will be awarded to the responsive and responsible vendor with the highest evaluation score as justified by procurement code.

The cost proposal requires vendors to submit an hourly rate for crisis communication services, with an estimated 20 billable hours per month historically used by the Division of Risk Management. The total approximate 5-year contract value is calculated using the formula: Hourly Rate × 20 Estimated Monthly Hours × 60 Months. Travel expenses may be incurred and will be reimbursed separately according to the Division of Finance Travel reimbursement policy. Vendors must complete only the yellow-highlighted field containing the hourly rate; any additional modifications to the cost proposal may result in rejection from the solicitation. The amount entered in the green "Total Approximate 5 Year Contract" box will be used for cost evaluation purposes.
