About this file

This document is an attachment to a Request for Statement of Qualifications (RFSQ) issued by the Utah System of Higher Education (USHE) through the State of Utah Division of Purchasing for broker and consulting services related to a consolidated health and welfare benefits program. The solicitation, numbered 07072026 and issued on July 7, 2026, seeks a qualified firm to design, procure, implement, and provide ongoing support for a consolidated employee health and welfare benefits program covering all USHE member institutions, with a target effective date of July 1, 2027. The document serves as Attachment B and contains a comprehensive questionnaire that vendors must complete to demonstrate their qualifications. Responses must address firm overview and licensing status, work history with comparable engagements including three detailed case studies, at least four client references with at least two from public-sector or higher education clients, and the firm's approach to strategic design, downstream vendor procurement, implementation, and ongoing account management. The RFSQ specifies that the selected firm will serve as the Board's broker and consultant of record and is expected to support the July 1, 2027 implementation date.

Vendors are explicitly prohibited from including any fee, cost, hourly rate, commission rate, or other pricing information in their responses, with inclusion of such information potentially rendering submissions non-responsive. The evaluation criteria are weighted as follows: Section A (firm overview and experience) at 20 percent, Section B (strategic design approach) at 20 percent, Section C (downstream vendor procurement and implementation) at 20 percent, Section D (key personnel and team composition) at 15 percent, Section E (compensation, transparency, and conflicts of interest) at 10 percent, Section F (troubleshooting and coordination with board-retained advisors) at 10 percent, and Section G (data management and security) at 5 percent. The firm selected as highest-ranked will proceed to negotiate compensation determined to be fair and reasonable, with the state retaining the right to proceed with the next-highest ranked vendor if negotiations with the top-ranked firm are unsuccessful. Vendors must disclose all current and recent business relationships with USHE member institutions and potential downstream vendors and must confirm willingness to execute Business Associate Agreements and Data Use Agreements prior to receiving protected health information.

Specifically identify any of your firm’s experience with: (i) multi-employer or consortium benefit programs; (ii) higher education clients; (iii) state or public-sector clients in Utah or in comparable jurisdictions; (iv) engagements involving the consolidation of previously independent risk pools; and (v) engagements involving an academic medical center as both an employer and a provider organization.
