Bid/Proposal Affidavit Summary

This is a Bid/Proposal Affidavit form required by the Maryland Judiciary's Administrative Office of the Courts for the Generator Maintenance contract opportunity spanning 2026 through 2031. The affidavit serves as a mandatory certification document that bidders must complete and submit as part of their proposal package. The form requires an authorized representative of the bidding business to affirm compliance with multiple Maryland state regulations and federal laws, certifying that neither the representative nor the business entity, its officers, directors, members, partners, controlling stockholders, or employees directly involved in contracting activities have been convicted of bribery, fraud, embezzlement, antitrust violations, or other specified criminal offenses. The document also requires affirmation regarding debarment status, collusion, contingent fee arrangements, and proper business registration with the Maryland State Department of Assessments and Taxation.

The affidavit contains nine major certification sections addressing bribery convictions, other criminal convictions, debarment history, related entity debarment, subcontractor debarment compliance, collusion in bidding, election law compliance for contracts exceeding $200,000 in cumulative consideration, corporation registration and tax payment status, and contingent fee prohibitions. Bidders must either affirm compliance with all requirements or provide detailed explanations of any inability to affirm, including specific dates, courts, charges, convictions, and current positions of involved individuals. The document acknowledges that the affidavit will be distributed to state and local government units, other states, and potentially federal agencies, and that false statements are subject to perjury penalties under Maryland and federal law.
