About this file
This file consists of four attachments (A1-A4) required for bid submission to Fairfax County, Virginia for Invitation for Bid (IFB) 2000004310 regarding the removal and replacement of three underground fuel storage tanks (UST). The solicitation requires bidders to submit sealed competitive bids for all labor, materials, tools, and equipment necessary to complete the project at all sites to be considered for award. The attachments establish mandatory certification and disclosure requirements that all bidders must complete and submit with their proposals, including safety violation history, business registration information, business classification designation, and ethics certifications.
Attachment A1 requires bidders to certify whether their firm has received any citations for safety violations within three years prior to bid submission, executed before a notary public. Attachment A2 mandates disclosure of Virginia State Corporation Commission (SCC) registration information or confirmation of exemption status for out-of-state entities. Attachment A3 requests voluntary business classification information, allowing bidders to self-identify as micro, small, non-profit, government, women-owned, minority-owned, service-disabled veteran-owned, or employment services organizations, though Fairfax County does not certify classifications or establish preferences. Attachment A4 requires an ethics certification whereby bidders confirm they have not provided improper payments or gifts to public employees with procurement responsibility, or if they have, that they received equal or greater consideration in exchange. All certifications must be signed by an authorized bidder representative with company identification information.
