About this file

This is a Limited Hazardous Materials Inspection report prepared by Terracon Consultants, Inc. for the State of Utah Division of Facilities Construction and Management (DFCM) regarding the Utah State Hospital Forensics Mechanical Room located at 1300 East Center Street in Provo, Utah. The inspection was conducted on May 6, 2026, in preparation for a planned mechanical room upgrade project (Project #27136420) with a construction cost estimate of $525,000. The approximately 3,500 square-foot facility comprises four rooms: the Chiller Room, Heat Exchanger Room, Electrical Room, and Upstairs Mechanical Room with interior finishes of CMU block, metal ceiling with fireproofing, and concrete or stone flooring. The inspection included comprehensive sampling and analysis for asbestos-containing materials (ACM), lead-containing paint (LCP), and universal hazardous wastes to identify materials requiring removal or special handling prior to renovation activities.

The inspection findings indicate that no asbestos-containing materials were identified in 24 bulk samples collected from seven homogeneous areas of suspect ACM and analyzed by Eurofins EMLab P&K using polarized light microscopy per EPA Method 600/R-93/116. However, lead was detected on two surfaces tested using X-ray fluorescence (XRF) analysis, with lead concentrations at or exceeding 0.01 mg/cm². Additionally, universal waste materials and other hazardous materials requiring proper disposal in accordance with EPA regulations were visually identified, with removal cost estimates provided in the report. The report notes that inaccessible areas within walls, voids, and concealed spaces may contain additional unsampled suspect materials and recommends that all building materials assumed to contain asbestos be removed by a Utah-certified abatement contractor prior to demolition or renovation activities to comply with federal OSHA requirements and Utah Division of Air Quality regulations.

1.0 Introduction

Terracon Consultants, Inc. (Terracon) conducted a pre-demolition limited hazardous materials inspection, including asbestos, lead, and universal hazardous materials, at the Utah State Hospital Forensics Mechanical Room located at 1300 East Center Street in Provo, Utah.

The inspection was conducted on May 6, 2026, by Mr. Noah Hancock. The objective of this project was to identify the presence of asbestos, lead, universal hazardous waste, and other hazardous materials present in preparation for possible renovation.

2.0 Building/Area Description

The Utah State Hospital Forensics Mechanical Room is roughly a 3,500 square foot area made up of four rooms: the Chiller Room, the Heat Exchanger Room, the Electrical Room, and the Upstairs Mechanical Room. The interior finishes consist of CMU Block, a metal ceiling coated in fireproofing, and concrete or stone flooring.

3.0 Field Activities

The hazardous materials inspection included asbestos, lead, and other hazardous materials and was conducted on May 6, 2026 by Mr. Noah Hancock, a Terracon employee and State of Utah-certified Asbestos Inspector. He is also trained to inspect for and summarize the presence of lead and universal hazardous wastes for proper disposal as required by the United States Environmental Protection Agency (EPA). Copies of Mr. Noah Hancock’s Asbestos and Lead Inspector certifications are attached as Appendix G.

3.1 Asbestos

Terracon began the asbestos sampling activities with a visual assessment and the identification and inventory of readily visible and accessible homogeneous areas of suspect ACM throughout the building. A homogeneous area consists of a material that appears similar throughout in terms of color, texture, and date of application. The assessment was conducted throughout visually accessible areas of the building. Materials identified as glass, metal, plastic, or rubber were not considered suspected asbestos containing material (ACM).

A physical assessment of each homogeneous area of suspect ACM was conducted to assess the friability and condition of the materials. A friable material is defined by the EPA as a material that can be crumbled, pulverized, or reduced to powder by hand pressure when dry. Friability was assessed by physically touching suspect material.

Based on results of the visual observation, bulk samples of suspect ACM were collected from the accessible interior spaces of the rooms. Bulk samples of suspected ACM were collected

May 21, 2026 | Terracon Project No. 61267098

Facilities | Environmental | Geotechnical | Materials 2 in accordance with Asbestos Hazard Emergency Response Act (AHERA) sampling protocols, using wet methods as applicable to reduce the potential for fiber release. Samples were placed in sealable containers and labeled with unique sample numbers, using an indelible marker. Terracon collected 24 samples from seven homogeneous areas of suspect ACM. A sampling location drawing is provided in Appendix B. Descriptions of the suspect materials, sample locations, and laboratory results are summarized in Table 1 Appendix A. Although reasonable effort was made to survey accessible suspect materials, additional suspect but unsampled materials could be in walls, in voids, underneath the concrete slab, or in other concealed areas.

Samples of suspect ACM were delivered under standard chain-of-custody protocol to Eurofins EMLab P&K (NVLAP #500031-0) in Phoenix, Arizona, for analysis by polarized light microscopy (PLM), using EPA Method 600/R-93/116. The asbestos content, where applicable, was determined by microscopic visual estimation. Copies of the analytical reports and chain-of-custody forms are provided in Appendix C.

3.2 Lead

The lead paint inspection began by visually surveying accessible interior room components, such as walls, ceilings, floors, doors, pipes, and other metal components. Various colors of paint were found on interior surfaces. All these components can be disturbed during renovation activities.

Terracon directly measured the concentration of lead using an Olympus X-ray Fluorescence (XRF) Lead Analyzer (Serial Number 510179). The Olympus XRF nondestructively measures lead concentrations in coated surfaces, regardless of the number of paint layers present. For this survey, materials were considered lead containing if the XRF indicated lead concentrations greater than or equal to 0.01 milligram per square centimeter (mg/cm2);

however, since the Occupational Safety and Health Administration (OSHA) does not permit materials to be classified as lead-free using an XRF, Terracon cannot guarantee that lead is not present in very low concentrations when the XRF indicates no lead was detected.

3.3 Hazardous Materials

Terracon visually identified hazardous wastes requiring removal and proper disposal prior to renovation or demolition by the Salt Lake County Health Department, but no samples of these materials were collected.

4.0 Regulatory Overview

4.1 Asbestos

The Utah Division of Air Quality (DAQ) requires an initial asbestos inspection or an asbestos re-inspection of buildings for which a previous asbestos inspection was conducted greater than three years prior. The DAQ enforces 40 CFR Part 61, Subpart M, the Asbestos National Emission Standard for Hazardous Air Pollutants (NESHAP) adopted by reference in Section 214-1, Utah Air Conservation Rules. The owner or operator must provide DAQ with written notification at least ten working days prior to the commencement of asbestos abatement activities that will disturb regulated asbestos-containing material (RACM) in amounts greater than or equal to 3 square feet or 3 linear feet.

The asbestos NESHAP regulates asbestos fiber emissions and asbestos waste disposal practices, and requires that RACM be identified, classified, and quantified prior to planned disturbances or demolition activities. The asbestos NESHAP regulation also requires the identification and classification of existing ACM according to friability prior to renovation or demolition activity. Friable ACM is a material containing more than 1% asbestos that, when dry, can be crumbled, pulverized, or reduced to powder by hand pressure. All friable ACM is considered RACM.

The asbestos NESHAP regulation classifies ACM as either RACM, Category I non-friable ACM, or Category II non-friable ACM. RACM includes all friable ACM; Category I non-friable ACM that has become friable or will be or has been subjected to sanding, grinding, cutting, or abrading; and Category II non-friable ACM that has a high probability of becoming or has become crumbled, pulverized, or reduced to powder in the course of demolition or demolition activity. Category I non-friable ACM are exclusively asbestos-containing packings, gaskets, resilient floor coverings, resilient floor covering mastics, and asphalt roofing products that contain more than 1% asbestos. Category II non-friable ACM are all other non-friable materials other than Category I non-friable ACM that contain more than 1% asbestos. Category II non-friable ACM generally includes but is not limited to glazing, mortar, grouts, and cementitious material such as pipes, siding, and panels.

The OSHA asbestos standard for construction (29 CFR 1926.1101) regulates workplace exposure to asbestos. The OSHA standard classifies construction and maintenance activities that could disturb ACM and specifies work practices and precautions that employers must follow when engaging in each class of regulated work. The standard also specifies requirements for disturbing and handling materials with asbestos concentrations less than or equal to one percent.

4.2 Lead Paint

The Lead in Construction OSHA Standard 1926.62 regulates construction work involving the presence of lead in any concentration. Depending on the specific construction activities

May 21, 2026 | Terracon Project No. 61267098

Facilities | Environmental | Geotechnical | Materials 4 being performed (torch cutting, manual demolition, sanding, grinding, etc.), employers must assume their employees are exposed to lead above the occupational exposure limit and must provide personal protective equipment, including respiratory protection, until an exposure assessment proves otherwise.

4.3 Hazardous Materials

The Environmental Protection Agency (EPA) has streamlined the regulations for certain categories of commonly generated hazardous waste, called universal waste, including:

Batteries

Pesticides

Mercury-containing equipment

Lamps

Aerosol cans

The EPA also has specific hazardous waste regulations for the following wastes:

Polychlorinated biphenyls (PCBs)

Chlorofluorocarbons (CFCs)

Drums or other containers of hazardous or special waste

5.0 Findings

5.1 Asbestos

Terracon collected 24 samples from seven homogeneous areas of suspect ACM from the site.

Based on the laboratory analytical reporting, no asbestos was identified in suspect materials sampled during this inspection.

Material descriptions, locations, and sample results are summarized in Table 1: Asbestos Bulk Sampling Summary in Appendix A. Laboratory analytical reports are included in Appendix C. A drawing depicting sample locations is provided in Appendix B.

Regardless of whether a material is considered a RACM by the State of Utah Division of Air Quality, federal OSHA asbestos regulations specify worker training, work practice, and exposure assessment requirements, among others, that most demolition contractors do not meet. Therefore, Terracon recommends that all building materials containing asbestos or assumed to contain asbestos be removed by a Utah-certified asbestos abatement contractor prior to demolition that would impact the materials.

Facilities | Environmental | Geotechnical | Materials 5

5.2 Lead Paint

Lead was detected in two surfaces tested. Results of surface coatings tested for lead are provided in Appendix E.

5.3 Hazardous Materials

A complete list of hazardous materials with removal costs is provided in Table 3 in Appendix F: Hazardous Waste Cost Estimates. These materials should be properly disposed of or recycled in accordance with all local, state, and federal regulations as part of future renovation or demolition activities.

5.4 Inaccessible Areas

Although reasonable effort was made to inspect accessible suspect materials, additional suspect but un-sampled materials could be in walls, in voids, or in other concealed areas.

The test report shall not be reproduced except in full, without written approval of the laboratory. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.

All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.

Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point count analysis to lower the detection limit and to aid in asbestos identification.
