Selection Criteria and Additional Requirements The following Selection Criteria will be used in evaluating each proposal. Each criterion is assigned a point value and weighed accordingly. The Selection Committee will consider all Selection Criteria in performing a comprehensive evaluation of the proposals. DFCM intends to enter into a Contract with the Prime Contractor providing the best value to the State as determined by the Selection Committee and according to the Selection Criteria.

Selection Criteria Points Summary Statement of Qualifications 30

Management Plan 25 Team’s Distance to Project Site 25

Past Performance References 20 Total Possible Points 100

A. Statement of Qualifications | 30 Points The Statement of Qualifications describes the experience and qualifications of the Contractor, and the project team members identified in the proposal. Include sufficient information in the Statement of Qualifications addressing each of the following:

• The lead contact person and other critical members of the team proposed for this project, including their qualifications and certifications required to provide inspections for this project;

• What the team will bring to the inspection process and how their knowledge and experience will benefit the project;

• Projects of comparable type, scope, and size previously completed by the team.

B. Management Plan | 25 Points The Management Plan describes how the project will be managed. Include sufficient information about the proposed Management Plan addressing each of the following Selection Criteria:

• The process used for managing sub-consultants such as special inspection and testing firms;

• A description of how the team will be organized, detailing the Contractor’s ability and plan to conduct inspections for the project;

• The method used to develop a schedule to ensure that inspections are completed;

• The addresses for each proposed inspector’s dispatch location to the project site;

• Any other information the Contractor deems necessary to assist the selection committee in evaluating the

Contractor’s management approach;

• An organizational chart of key personnel with a description of their responsibilities.

C. Team’s Distance to the Project Site | 25 Points Provide both the testing lab location and special inspector’s distances to the project site from their dispatch location (one way). DFCM is not asking Contractors to waive travel charges, nor are travel charges part of the scoring criteria. The importance of considering the teams distance to the project site relates to response time as inspections are requested.

D. Past Performance References | 20 Points DFCM Performance Ratings Special Inspections and Testing Contractors are required to fill out and include in their proposal the provided DFCM Past Performance References form. Contractors must provide projects that are of similar scope and size, and that are no more than five years past completion. All references must be for projects in which the Contractor was the prime Contractor. These projects can be for both DFCM projects and non-DFCM projects.

DFCM will contact the references provided and ask them to rate the contractors performance on a scale of 1 (low) to 5 (high) in multiple areas. Ratings will be averaged and used in the selection scoring matrix.

Complete and accurate information for non-DFCM references must be provided on the DFCM Performance References Form. The form requires a minimum of five references. Incomplete reference information or the

DFCM VBS Special Inspections and Testing - Request for Proposals, 1.15.2026 8 inability for DFCM to acquire a rating from a provided reference may result in the Contractor being disqualification from the selection process.

Additional Requirements The following documents are not part of the Selection Criteria but are requirements that, if not met, may disqualify the Contractor from the selection process or award of the Contract if not submitted.

E. DFCM Termination or Debarment Form The Contractor must sign and submit the provided DFCM Termination or Debarment Form certifying that neither it nor its principals are presently debarred, suspended, proposed for debarment, declared ineligible, or voluntarily excluded from soliciting work by any government agency. This form also certifies that neither they nor their principals have been terminated during the performance of a contract or withdrew from a contract to avoid termination. If the Contractor cannot certify these two statements, the form provides a section for a written explanation of the circumstances to be provided for review by DFCM. Although this form is not part of the Selection Criteria, it is a required proposal document. Proposals submitted without this form may lead to Contractor disqualification.

F. Health Insurance Statement of Compliance Form (applicable to the awarded Contractor only) The Health Insurance Statement of Compliance Form applies to: (i) a prime Contractor (and design professional) if the prime contract (or design professional contract) is in the aggregate amount of $2,000,000 or greater; and

(ii) applies to a subcontractor (and subcontractor of design professional) if the subcontract is in the aggregate amount of $1,000,000 or greater. For benchmark information, please visit Health Insurance Statement of Compliance Benchmark Information.

If applicable to this project, the awarded Contractor must submit the Health Insurance Statement of Compliance Form to DFCM via email at dfcmcontracts@utah.gov before the Construction Contract Notice to Proceed will be issued. The form can be found on the DFCM website – Construction Management - Standard Documents page and is also posted in U3P.
