Date/Time for Receipt of Proposals: no later than November 16, 2020 at 2:00 p.m. Central Standard
Time.
Date/Time for Receipt of Pre-Proposal Questions: Submit all questions in writing to lauree.morris@gsa.gov no later than October 29, 2020 at 5:00 p.m. Central Standard Time.
L.2.1 INFORMATION TO BE INCLUDED IN TECHNICAL PROPOSAL
In order to receive consideration in the technical evaluation process, the Offeror’s technical proposal must address all of the areas outlined in Section M, Item M.2. Simply repeating or paraphrasing the Statement of Work/Specifications is not acceptable.
Offerors shall submit proposals that are acceptable without additional explanation or information. The
Government may request additional information from Offerors of proposals and may discuss proposals with the Offerors. However, award may be made without discussion or negotiation of proposals.
The contents of the Technical Proposal shall consist of:
1. Work Experience
2. Past Performance
3. Quality Control Plan (QCP)
4. Site Visit Record
PAST PERFORMANCE SURVEYS
Three Past Performance Surveys (see M.9) are required as part of the technical package and are due no later than the proposal due date and time. It is the Offeror’s responsibility to ensure that a survey response from each of the Offeror’s three references is provided directly to the Contracting Office in accordance with M.9 instruction. Three (3) past performance surveys must be included from three (3) different contracts. The same contracts used for the experience references submitted under the Work Experience
Factor shall be used for the Past Performance Factor. If more than three references are submitted, the
Government will only evaluate the first three references submitted.
If less than three references are submitted, the Offeror will be scored zero for those references unsubmitted. In the event an Offeror without a record of relevant past performance is not available, the
Offeror will be rated neutral.
L.2.2 INFORMATION TO BE INCLUDED IN PRICE PROPOSAL
The contents of the Price Proposal shall consist of:
1. Signed copies of the Standard Form 1449
2. Completed pricing and other information in Section B

Solicitation 47PG0120R0008 Page 137 of 145 performance. The Government reserves the right to use reference sources the Government is aware of, or becomes aware of, even if not submitted by the offeror.
Standard for Evaluation: The standard is met when the offeror, as a prime contractor, submits three past performance surveys (for the same three contracts used for Factor 1, Work Experience) in performing similar services for each contract with a minimum of two (2) years continuous satisfactory performance within the last five (5) years.
The offeror’s performance on similar contracts must be rated good or better on each evaluation factor in the performance level chart on the past performance survey, no significant or recurring deficiencies, no unnecessary proposal to increase contract cost, and yes to both backup staffing and willingness to contract with the offeror again. Performance level indicating very good or excellent for the various evaluation factors is value added.
Each survey submitted must provide the following information and shall be submitted on the Past
Performance Survey forms (Item M.9 – this is a two page document).
1. Contractor Name
2. Contract/Account Number
3. Agency/Company Name and location
4. Agency/Company Contact Name and Valid Phone Number (Contracting Officer and Building
Manager)
5. Performance History
The past performance survey (M.9, THIS IS A TWO (2) PAGE DOCUMENT) must be in a sealed envelope from the original evaluator with their signature and date across the seal of the envelope.
NOTE: You must submit the past performance surveys to your references and inform them to fill out the survey and send it back to you inside of a sealed envelope.
● YOU CANNOT OPEN THE SEALED ENVELOPES
● THE SEALED ENVELOPES MUST BE FORWARDED WITH YOUR PROPOSAL.
IT IS THE OFFERORS RESPONSIBILITY TO RETURN THE THREE (3) SURVEYS WITH THEIR
PROPOSAL PACKAGE.
FACTOR 3 – QUALITY CONTROL PLAN Weight - 20%
Description: This factor considers the effectiveness, efficiency, and soundness of the Offeror’s quality control system. It considers whether the quality control system encompasses all aspects of the process prior to and concurrent with providing the services. The Quality Control Plan (QCP) shall include the following:
Staffing Plan This shall include:
● Equipment List
● Names and job titles of proposed staff
● Identify roles and responsibilities for oversight of the QCP and functions associated with such oversight
● Point of contact name, phone number, and email address for company representative who will deal with the contract and any associated task ordersResumes and certifications for all proposed supervisors. Supervisors must meet the qualifications in Item H.1.1
