About this file
This is a Question and Answer document from the Nevada Department of Corrections (NDOC) regarding a telemedicine services contract (44DOC-S3645). The contract calls for the provision of remote clinical services across three correctional facilities: High Desert State Prison (HDSP, 2,745 inmates), Lovelock Correctional Center (LCC, 1,543 inmates), and Northern Nevada Correction Center (NNCC, 1,345 inmates). The contractor must provide telemedicine medical services operating Monday through Friday from 8 a.m. to 5 p.m., with a break around 11 a.m. for count. Expected clinic volumes include approximately 20 daily visits at HDSP (375 monthly including intake), 10-12 daily visits at LCC, and 30 daily visits at NNCC (600 monthly). The contractor must supply a cart-based kiosk with medical peripherals including a blood pressure cuff, telehealth software, and maintain their own EHR system with capability to transmit consult notes to NDOC. Chronic care services are required for conditions including congestive heart failure, seizure disorder, diabetes mellitus, thyroid disease, hepatitis C, HIV, hypertension, COPD, and chronic kidney disease. The contractor must be able to follow NDOC's medication formulary and have access to CIPS (the state's corrections information system).
The contractor is expected to manage clinic staffing with remote telemedicine providers who will see scheduled patients independently, supported by an onsite facilitator to screen patients prior to consultation. No after-hours or urgent care coverage is required. NDOC will provide onsite nursing staff assistance but the goal is for visits to occur without requiring onsite staff during the actual consultation. Currently, 3,264 patients are enrolled in chronic care services, and NDOC receives approximately 140 health service requests (kites) daily across all facilities. Performance success metrics focus on improving clinical outcome measures such as blood pressure and A1C levels while decreasing emergency room visits for preventable conditions. In-person care escalation occurs through onsite providers at main facilities, with the telemedicine provider able to refer patients back to facility providers for additional assessment when necessary.

| 7 | AUSTINSCHULTEA | Process for in-person care escalation | If a patient requires in-person evaluation or treatment by a medical provider that cannot be completed via telemedicine, please describe NDOCâ??s current process and infrastructure for providing in-person medical care.
