About this file

This document is an R2 Audit questionnaire for RIOS (Recycling Industry Operating Standard) certification covering multiple prison facilities operated by Unicor. The audit covers seven sites: Allenwood/Schuylkill, Atwater, Leavenworth, Marianna, Texarkana, and Terminal Island. Each site is R2V3 certified with scopes focused on electronic equipment processing, including downstream vendor management, physical and logical data sanitization, testing, materials recovery, and selling electronic equipment and components for reuse. All sites employ manual dismantling for material recovery, have no major audit findings (only a few minor findings that were closed), and have all Tier 1 downstream vendors who are R2V3 certified. The RIOS and R2 management systems are integrated, and the organization is not interested in virtual audits or requiring translation services.

Questions for R2

Per site were there any Findings (major or minor) at last audit? No major findings at any sites. Only a couple of minor findings for the entire group and all have been closed.

Per site Downstream Recycling Chain: (Counting of DSV by counting the non-r2s up to the first R2v3 in the stream. Thus if Tier 1 for circuit boards is R2v3 certified, don’t count any further.) Per facility please list which of these three categories do they fall under. All Tier 1’s are R2V3.

R2v3 Certified DSV only, 1-5 total of Non R2v3 DSV (to the first R2v3 from each FM stream or through end of life, whichever occurs first), 6+ total of Non R2v3 DSV (to the first R2v3 from each FM stream or through end of life ever occurs first. All Tier 1’s are R2V3.

Per site Material Recovery Manual dismantling or Other? Manual dismantling at all sites.

Questions for both standards:

Are there any sites in which translation will be needed? No

Are you interested in virtual audits and have the capability to conduct such an audit? No

Are the RIOS and R2 management systems integrated? Yes
